手用ProTaper在后牙残根残冠中的临床应用研究

目的：探讨手用ProTaper在后牙残根、残冠保存治疗中的应用效果。方法：102例病人随机分为2组，对照组50例，采用普通不锈钢器械进行根管预备，多次法完成根管治疗术，观察1周无症状后加根管钉，树脂或银汞合金修复；实验组52例，采用手用ProTaper预备根管，其他步骤与对照组相同。分析两种不同器械根管预备时间、术中、术后疼痛发生率、根充效果、断针率以及桩修复的操作难度和效果。结果：两种不同器械根管预备时间、术中、术后疼痛发生率差异无统计学意义（P〉0．05）；ProTaper预备根管恰填率较普通器械高（P〈0．05），但断针率也高（P〈0．05）；用常规器械预备、根充后的牙齿，加根管钉，出现根管钉偏移6例，而ProTaper预备根充后，加根管钉，出现根管钉偏移只有1例（P〈0．05），两组均未出现侧穿。结论：手用ProTaper在后牙残根、残冠保存治疗中与普通器械预备效率、术中、术后疼痛反应发生率相似，断针率高，但恰填率高于常规器械组，而且加根管钉不易发生偏斜、侧穿。

Clinical study of ProTaper nikel-titanium hand files in management of severely-decayed posterior tooth

AIM:To explore the effect of ProTaper nikel-titanium hand files on the management of severely-decayed posterior teeth.METHODS: One hundred and two severely-decayed posterior teeth were divided into two groups randomly.In group A,50 teeth were instrumented with stainless steel K file(SS group);in group B,52 teeth with ProTaper nikel-titanium hand files(PT group).One week after root canal therapy the teeth were completed with multi-times method,the selected root canals were placed with the post,and the teeth were restored with amalgam or resin materials.The time of preparation and pain reaction,obturation effect,instrument fracture and post preparation were recorded.RESULTS:Both groups had no difference in the time of preparation and pain reaction(P>0.05),but ProTaper nikel-titanium hand files had good obturation effect and was apt to post restoration.It was noted that ProTaper had high incidence of instrument fracture.CONCLUSION:ProTaper nikel-titanium hand files has the similar results in time of preparation and pain reaction compared with stainless steel K file.But ProTaper nikel-titanium hand files has good obturation effect and is apt to post restoration.
