快速成型机在骨盆骨折治疗中的应用

摘    要:目的探讨快速成型技术在骨盆骨折中的应用价值。方法骨盆骨折Tile分型C型旋转不稳定性骨折42例患者,其中男性24例,女性18例;年龄17~54岁。根据入院顺序随机分为2组,每组21例。其中试验组男性13例,女性8例,平均年龄37.3岁;对照组男性11例,女性10例,平均年龄35.5岁。两组间损伤严重度评分(ISS)差异无统计学意义。对试验组骨盆进行快速成型建模,在模型上模拟手术操作后再进行实际手术;将术中指标与未进行建模模拟手术的对照组指标进行比较。结果试验组和对照组的手术时间分别为(113.4±12.3)min和(159.3±47.4)min,两组间差异有统计学意义(P〈0.05)。试验组术中出血量和输血量分别为(762.3±87.4)mL和(503.3±43.4)mL,而对照组分别为(972.1±75.4)mL和(862.3±81.7)mL,两组间差异有统计学意义。试验组复位满意率为90.5%,而对照组为85.7%,两组间差异无统计学意义(P〉0.05)。试验组术后功能评分优良率95.2%,对照组优良率90.5%,两组比较差异无统计学意义(P〉0.05)。结论快速成型技术可有效缩短复杂骨盆骨折的手术时间,减少术中创伤,对骨盆骨折的治疗具有很好的指导意义。

Abstract:Objective To investigate the value of application of rapid prototype technology in therapy of pelvic fractures. Methods A total of 42 patients with C-type rotary unstable pelvic fracture was enrolled, male 24, female 18, aged 17 - 54 years old, which randomly divided into 2 groups by hospital order, each of 21: male 13, female 8, mean aged 37.3 years old in test group; male 11, female 10, mean age 35.5 years old in control group. The injury severity score(ISS) was no significant difference between 2 groups. The control group was treated without the aids of rapid prototype technology. Test group was treated with the help from rapid prototype technology and the mimic operations were performed before practical operation. The operative time, blood loss, blood transfusion and reduction satisfaction of both groups were compared. Results The operative time, blood loss and blood transfusion in test group[(113.4 ±12.3) minutes, (762.3 ±87.4) mL, (503.3 ±43.4) mL, respectively)] was lower than that of the control group [(159.3 ±47.4) minutes, (972.1 ±75.4) mL, (862.3 ±81.7) mL, respectively]. There was no significant difference in reduction satisfaction and postoperative functional evaluation between 2 groups (P 〉 0.05). Conclusion It is demonstrated that application of rapid prototype technology in the therapy of pelvic fractures could reduce both operative time and the intro-operative trauma.
