嗜铬细胞瘤20例临床分析

目的：了解嗜铬细胞瘤的发生特点,以指导临床诊断与治疗。方法：回顾分析20例我院近7年来出院诊断为嗜铬细胞瘤患者的临床特点。结果：20例嗜铬细胞瘤患者中,经手术病理检查证实19例;肿瘤来源于肾上腺17例,肾上腺外3例（1例位于左肾门,1例位于膀胱壁,1例为多发）;单发肿瘤19例,其中意外瘤6例（30%）;多发肿瘤1例,来源于左腹主动脉与肾门间、右肾下极与下腔静脉间。临床上伴血压升高18例（90%）;有头痛、心悸、多汗三联征2例（10%）,分别有头痛、心悸者各7例（35%）,有多汗者5例（25%）;伴高血糖11例（55%）。血儿茶酚胺检查的阳性率为76.9%（10/13）;24h尿3-甲氧基-4-羟基苦杏仁酸（VMA）测定阳性率50%（3/6）。意外瘤患者至少有一项血儿茶酚胺或24h尿VMA异常,占50%（3/6）。结论：临床表现结合血儿茶酚胺和尿VMA测定,并辅以影像学手段对嗜铬细胞瘤的诊断是非常重要的,但最终仍需依赖病理诊断。意外瘤生化检查可以提高其检出率,临床需仔细甄别,以免漏诊。

Clinical retrospective analysis of 20 cases of pheochromocytosis
