卵巢上皮癌化疗药物敏感试验的临床研究

摘    要:目的:探索体外化疗药物敏感试验对卵巢癌临床用药的指导作用。方法:将卵巢上皮癌患者的肿瘤细胞体外培养,经10种、20个不同浓度化疗药物作用,用活细胞荧光探剂标记,在激光扫描共聚焦显微镜(CLSM)下测定药物作用前后的活细胞数量。结果:标本可评价率为80.95%,预测结果的临床阳性符合率为89%,阴性符合率断100%。82.35%的标本对泰素敏感,泰素、顺铂、卡铂和足叶乙甙具有显著的剂量效应关系。用敏感试验指导治疗的患者,无瘤生存时间较对照组长,P<0.05。结论:CLSM法体外化疗药物敏感试验,准确直观,细胞用量少,对临床用药有指导意义。

Abstract:Objective:To study the feasibility and validity of drug sensitivity assay in epithelial ovarian cancer by confocal laser scanning microscopy (CLSM). Methods: Ovarian cancer specimens were obtained from 21 patients with epithelial ovarian cancer. Tumor cells exposed to ten agents, each of which has two concentrations. After 72 hours, 6- carboxy-fluorescein-diacetate (CFDA) was added. The tumor cells were calculated by CLSM and video recording. Statisticly Q test and analysis of multiple variance were applied. Results: With the patients classified as sensitive to the drug in vitro assay results, their tumor cells were sensitive to at least one of the agents used in their primary chemotherapy regimen. The results showed a true positive test rate of 89% and a true negative test rate of 100%. The patients who were classified as sensitive had a longer disease-free interval compared with those patients who were not, P<0.05. Conclusions: The CLSM may be a simple and rapid method of chemosensitivity and it may prove to be clinically significant.
