门诊药房调剂差错原因分析与防范措施

目的:分析门诊药房调剂差错的原因,杜绝药疗事故发生,提高药品调配工作质量。方法:回顾性分析2004～2008年我院80例门诊处方调配差错情况及产生的原因,采取相应防范措施,建立规范调剂操作规程与行之有效的监督管理机制。结果:门诊处方调配差错原因主要有四大类型:医师处方书写不规范23例,占28.75%,因数量差错16例,占20%,因药物品种差错15例,占18.75%,用法差错13例,占16.25%。结论:通过对门诊药房发药差错的分析,采取防范措施和规范调剂规程,有效的管理制度和人员素质的培养,最大限度的预防差错发生。

Causes for errors of drug allocation in outpatient pharmacy and its prevention

Objective： To analyze causes for errors of drug allocation in outpatient pharmacy and its prevention and to improve drug allocation efficacy. Methods： 80 cases with errors of drug allocation in outpatient pharmacy of our hospital during 2004 and 2008 were retrospectively analyzed, and corresponding preventive measures such as proper working procedure and supervision measures were carried out to improve it. Results： Errors of drug allocation in outpatient pharmacy can be attributed to four main causes： doctors irregular handwriting（23, 2 75%）, wrong usages （13, 16.25% pharmacy can help to improve drug 8.75%）, wrong dosages （16, 20%）, mistaken drug names （15, 18. ）.Conclusion： Analysis of the errors of drug allocation in outpatient allocation efficacy and staffs professional skills.
