螺旋CT对结核性腹膜炎的诊断价值分析

目的:对结核性腹膜炎(Tuberculous peritonitis,TBP)的CT检查进行分析研究,以提高临床诊断TBP的水平。方法:回顾性分析27例有CT扫描资料的TBP病例,其中经手术病理证实3例,经腹腔穿刺和(或)经临床综合指标及抗结核治疗随访显效而诊断者24例,观察TBP累及腹膜腔及腹膜后的CT表现特征。结果:腹水25例,占本组病例的92.6%(25/27),其中中量、少量腹水以下腹部为主的19例,占本组腹水的79.2%(19/24);肠系膜改变23例,占本组病例的85.2%(23/27),表现为增厚、结节状、污秽状;网膜改变12例,占本组病例的44.4%(12/27),表现为污秽状及饼状网膜;腹膜增厚13例,占本组病例的48.1%(13/27);淋巴结肿大及钙化共6例,占本组病例的22.2%(6/27)。结论:螺旋CT检查作为临床诊断TBP的辅助手段,具有一定的诊断价值。

Analysis of the diagnostic value of spiral CT to the tuberculous peritonitis

Objective To analysis tuberculous peritonitis by using CT examination and to improve the clinical diagnosis of tuberculosis peritonitis level.Method 27 cases of CT scanning data of tuberculous peritonitis were analyzed retrospectively,of which 3 cases were confirmed by operation and pathology.24 cases were diagnosed by trans abdominal puncture and(or) by clinical comprehensive index and the follow-up effect of antituberculous therapy.Observed the CT features of TBP involving the peritoneal cavity and retroperitoneal.Results Ascites were in 25 cases,92.6% in this group(25 /27),among them,a small amount of ascites which were mainly following abdominal consisting were 19 cases,The ascites were 79.2% in this group(19/ 24);Mesenteric change was in 23 cases,accounting for 85.2%(23 /27)in this group,manifested as thickening,nodular,filth;Retinal change was in 12 cases,accounting for 44.4%(12 /27) in this group,manifested as filth and pie omentum;Peritoneal thickening was in 13 cases,accounting for 48.1%(13 /27) in this group;Lymphadenectasis and calcification was in 6 cases,22.2%(6 /27)in this group.Conclusion Spiral CT examination has a certain value as auxiliary means in diagnosing TBP.
