脑干缺血患者的脑干听觉诱发电位研究

目的 :研究早期怀疑脑干腔梗 (或小片状梗死 )患者的脑干听觉诱发电位 ( BAEP)变化。方法 :对 2 0例临床以椎一基底动脉缺血为主患者进行 BAEP检查并以 2 0例正常人做对照。结果 :早期怀疑脑干腔梗 (或小片状梗死 )患者中 ,1 3例 BAEP异常 (占 65% ) ,其中 6例经 CT证实为脑干小片状梗死 ,6例经 MRI证实为脑干腔梗 ,这些腔梗和小片状梗死多涉及桥脑被盖侧方 ,1例 MRI未见异常 ,7例 BAEP均属正常范围 (占 35% ) ,其中 CT示桥脑小片状梗死者 3例 ,MRI示桥脑腔梗者 3例 ,这些脑干腔梗和小片状梗死多接近中线部 ,MRI正常者 1例。结论 :BAEP图形的异常改变 ,是脑干梗死患者早期变化的一个指征 ,而正常 BAEP不能排除脑干腔梗或小片状梗死。

Study of Brainstem Auditory Evoked Potential (BAEP) in patients with brainstem ischemia

Objective:To study the BAEP changes of the patients with suspicious early brainstem lacuna infarction or patch infarction.Methods:The BAEP were tested in 20 patients with clinically vastibulo-basaler insufficiency and 20 healthy subjects as the controls.Results:13 patients(65%) had abnormal BAEPs. Among them, 6 patients were documented by CT as brainstem patch infarction, 6 patients were documented by MRI as brainstem lacuna infarction and one patient had no abnormal MRI finding,most of the lesions were in the lateral sides of the pontine tegmentum, 0ther 7 patients (35%) had normal BAEPs, among them, 3 patients were documented by CT as pontine patch infarction, 3 patients were documented by MRI as pontine lacuna infarction and one patient had no abnormal MRI finding, most of the lesions localized near the pontine midline.Conclusion:The abnormality of BAEP is an early indication of brainstem infarction,but in patients with normal BAEP,lacuna or patch infarctions could not be excluded.
