糖尿病对大肠癌术后并发症的影响

目的 探讨大肠癌围手术期糖尿病的影响及处理。方法 对６３例大肠癌并存糖尿病患者围手术期诊断与治疗进行回顾性分析。结果 ６３例病人均行择期手术，糖尿病以２型多见，术后并发症发生率；切口感染１３例，吻合口瘘３例，骶前积脓２例，肺部感染２例，腹腔脓肿１例，泌尿系感染３例，高渗性非酮症昏迷２例并随后死亡，酮症酸中毒１例，术后平均住院２３ｄ。

Influence of diabetes mellitus on operation of large bowel cancer

Objective To study the effect and management of diabetes mellitus in perioperative period of large bowel cancer. Methods 63 cases of diabetic patients with large bowel cancer underwent varous surgical procedured were analyzed retrospectively.Results All 63 cases received selective operations. The associated diabetes occurred frequently in type 2. The morbidity of postoperative complications: 13 cases of wound infection (20.6%), three cases of anastomotic leak, two cases of presacral space abscess, two cases of pneumonia, one cases of intra-abdominal abscess, three cases of urinary tract infection, two cases of hyperosmolar coma who finally died and one case of postoperative diabetic ketoacidosis. The patients in wards after operation were mean 23 days. Conclusion This study shows that diabetes mellitus can increase postperative complications. If the patient's blood-glucose concentration can be well controlled and operations are selected reasonable, it will be benefitial and safe to the patients in the perioperative period.
