脑室─腹腔分流治疗脑积水首次手术失败62例分析

目的：对脑积水病例首次脑室一腹腔分流水（VP）失败原因进行分析。方法：以VP治疗各类脑积水320例，其中62例首次手术失败，结合脑积水的成因，对败原因进行分析和统计学处理。结果：分流管阻塞是多见，占69．4％（43／62），其中又以近端管阻塞多见，占阻塞病例的62．8％（27／43）。失败率高低顺序依次为：先天性、炎症性、出血性、肿瘤性、外伤性。失败率差异高度显著。结论：手术失败主要与操作和病例及手术时机的选择有关。手术应由有经验的专科医生进行。小儿脑积水，若有严重的脑发育不良，手术失败率高。

Analysis of the 62 First-time Failed Operations in Hydrocephalus Treated by Ventriculoperitoneal Shunt

Objictive:Discuss the failed reasons of the first time ventrieuloperitoneal shunt in hydrocephalus patients,Methods:320 cases of hydrocephalus were treated by ventriculopertoneal shunt,62 cases were failed in the first time.Combined theetiology of hydroeephalus,the failed reasons of the operation were analysed,Results:The obstruction of the tube was most seen(69.4%,43/62),and most of them were the obstruction of the near part(62.8%,27/43).According to the occuring frequeney,the failed reasons are congenital,infection,hemorrhang,tumor ,trauma,The results are obviouse difference between the operations with different reasons.Conclusion:The results of operation are related to the operation technique and selected cases.The operation must be done by expert doctor,The falied ration is high in hydrocephalkus patient with brain dysplasia.
