血清甲状腺球蛋白联合131I全身显像在分化型甲状腺癌病情评估中的临床价值

目的 探讨甲状腺球蛋白（Tg）联合治疗剂量131I全身显像（131I-WBS）在已行131I清除残余甲状腺组织的分化型甲状腺癌（DTC）患者中监测肿瘤复发或转移的临床价值.方法 66例接受甲状腺全切或大部切除并已行131I清除残余甲状腺治疗的DTC患者,进行血清Tg测定和治疗剂量的131I全身显像.结果 对DTC复发或转移灶诊断的灵敏度、特异度和准确性：Tg分别为78.7%、84.2%和80.3%;131I-WBS分别为85.1%、100.0%和89.4%;Tg联合131I-WBS分别为91.5%,84.2%和89.4%.结论 血清Tg测定联合治疗剂量的131I全身显像在已行131I清除残余甲状腺治疗的DTC患者病情评估中有互补作用,对监测复发和转移病灶有重要的临床价值.

The clinical value of serum thyroglobulin combined with 131I whole body scan in severity evaluation patients with differentiated thyroid cancer

Objective Tostudy the clinical value of serum thyroglobulin（Tg） combined with therapeutic dose 131I whole body scan （ 131I - WBS） in treating patients with differentiated thyroid cancer（ DTC ） as the monitor index of neoplasm recurrence or metastasis after 131I radiotherapy. Methods Sixty - six patients with DTC after radical or partial thyroidectomy and 131I radiotherapy were applied with serum Tg measurement and posttreatment 131I- WBS. Results Among 66 patients, the sensitivity of Tg and 131I - WBS in diagnosis of DTC recurrence or metastatic lesion was 78.7% and 85.1% , respectively, and the specificity was 84.2% and 85.1% , respectively; the accuracy was 80. 3% and 89.4%, respectively. The total sensitivity, total specificity and total accuracy of combination Tg with 131I - WBS was 91.5 %, 84. 2% and 89.4% , respectively. Conclusion Serum Tg measurement combined with therapeutic dose 131I - WBS in patients with DTC for monitoring recurrence and metastatic lesions has important clinical value.
