AFU、AFP-L3％和GP73在原发性肝癌中的应用价值

目的探讨α-L-岩藻糖苷酶(AFU)、甲胎蛋白异质体L3比率(AFP-L3%)和高尔基体跨膜糖蛋白73(GP73)在原发性肝癌(PLC)诊断、疗效观察和复发监测中的应用价值。方法选取71例PLC患者为观察组,其中行手术、射频消融治疗的36例为手术组;经肝动脉栓塞化疗的31例为化疗组;治疗3~6个月后复发的47例为复发组;同时选取同期健康体检者80例为对照组。比较各组AFU、AFP-L3%和GP73水平;比较AFU、AFP-73%和GP73单独及联合检测诊断PLC的灵敏度和特异度。结果观察组治疗前、化疗组治疗后、复发组AFU、AFP-L3%、GP73水平均高于对照组(P<0.05)。手术组治疗后AFU、AFP-L3%和GP73水平低于观察组治疗前(P<0.05),化疗组治疗后AFP-L3%和GP73水平也较观察组治疗前下降(P<0.05)。与观察组治疗前和复发组比较,手术组和化疗组治疗后AFU、AFP-L3%和GP73的阳性率均下降(P<0.05)。GP73单独检测诊断PLC的灵敏度和特异度高于AFU和AFP-L3%单独检测(P<0.05);AFU、AFP-L3%和GP73联合检测诊断PLC的灵敏度与特异度高于各项单独检测(P<0.05)。结论血清AFU、AFP-L3%和GP73水平在PLC的诊断、疗效观察和复发监测方面具重要的应用价值。

Application value of AFU,AFP-L3% and GP73 in primary liver cancer

Objective To investigate the value ofα-L-Fucosidase(AFU),alpha fetoprotein heterogeneity L3 ratio(AFP-L3%)and Golgi transmembrane glycoprotein 73(GP73)in the diagnosis,curative effect observation and recurrence monitoring of primary liver cancer(PLC).Methods A total of 71 patients with PLC were selected as the observation group,36 patients who underwent operation and radiofrequency ablation as the operation group,31 patients who underwent transcatheter arterial chemoembolization as the chemotherapy group,47 patients who recurred 3 to 6 months after treatment as the recurrence group,and 80 healthy people as the control group.The levels of AFU,AFP-L3%and GP73 were compared,and the sensitivity and specificity of AFU,AFP-73%and GP73 alone and combined detection for diagnosis of PLC were compared.Results The levels of AFU,AFP-L3%and GP73 in the observation group before treatment,chemotherapy group after treatment and recurrence group were higher than those in the control group(P<0.05).The levels of AFU,AFP-L3%and GP73 in the operation group after treatment were lower than those in the observation group before treatment(P<0.05),and the levels of AFP-L3%and GP73 in the chemotherapy group after treatment were lower than those in the observation group before treatment(P<0.05).Compared with the observation group before treatment and the recurrence group,the positive rates of AFU,AFP-L3%and GP73 in the operation group and chemotherapy group after treatment were decreased(P<0.05).The sensitivity and specificity of GP73 diagnosis PLC were higher than AFU and AFP-L3%separate detection(P<0.05);the sensitivity and specificity of AFU,AFP-L3%and GP73 combined detection diagnosis PLC were higher than the 3 indexs separate detection(P<0.05).Conclusion Serum AFU,AFP-L3%and GP73 levels are of great value in the diagnosis,curative effect observation and recurrence monitoring of PLC.
