人工关节置换术后假体周围感染病原菌分布及耐药性分析

摘    要:目的分析该院2016-2018年人工关节置换术后患者假体周围感染的常见病原菌分布及耐药性,为临床治疗合理选用抗菌药物提供依据。方法对该院2016-2018年接受人工关节置换术患者的817份伤口分泌物标本和282份关节腔液标本进行病原菌分布与耐药性统计分析。结果伤口分泌物标本中共分离出病原菌214株,阳性检出率为26.19%;关节腔液标本中共分离出病原菌65株,阳性检出率为23.05%。药敏结果显示,金黄色葡萄球菌是伤口分泌物标本和关节腔液标本中主要菌种,未出现耐万古霉素的金黄色葡萄球菌。结论人工关节置换术后患者伤口分泌物和关节腔液标本分离的细菌种类较多,以金黄色葡萄球菌为主,掌握其分布情况、耐药性变化,有助于合理选择抗菌药物。

Abstract:Objective To analyze the periprosthetic infections in patients undergoing artificial joint replacement in our hospital from 2016 to 2018,so as to provide basis for the rational use of antibacterial drugs.Methods The distribution and drug resistance of 817 wound secretions and 282 cavity fluid specimens from patients undergoing artificial joint replacement were analyzed.Results A total of 214 strains of pathogenic bacteria were isolated from wound secretion samples.The detection rate was 26.19%.A total of 65 strains of pathogenic bacteria were isolated from articular cavity fluid samples.The detection rate was 23.05%.Staphylococcus aureus was the main strain in the wound secretions and cavity fluid specimens.There were no Staphylococcus aureus strains resistant to vancomycin.Conclusion In patients undergoing artificial joint replacement,the bacteria isolated from wound secretion and articular cavity fluid were diversified,which is mainly of Staphylococcus aureus.Knowing the distribution of bacteria and drug resistance is helpful to the rational use of antibacterial drugs.
