卵巢上皮癌手术癌组织蛋白激酶B及术后患者血清胸苷激酶1表达与预后的相关性分析

摘    要:目的检测卵巢上皮癌患者癌组织蛋白激酶B(PKB)、术后血清胸苷激酶1(TK1)水平,并探讨其表达水平与预后的相关性。方法选取2013年1月至2015年1月于张家口市第一医院住院并在术后经病理证实的60例卵巢上皮癌患者(卵巢上皮癌组)和60例卵巢良性肿瘤患者(良性肿瘤组)作为研究对象。采用免疫组织化学法检测癌组织中PKB水平,化学发光数字成像分析仪检测术后患者血清TK1表达;分析癌组织PKB和术后血清TK1表达与卵巢上皮癌临床病理参数的关系;分析癌组织PKB、术后血清TK1表达与卵巢上皮癌患者生存率的关系;COX单因素和多因素分析判别卵巢上皮癌预后独立危险因素。结果卵巢上皮癌组癌组织PKB蛋白阳性表达率和术后血清TK1水平均显著高于良性肿瘤组,差异具有统计学意义(P<0.05);癌组织PKB和术后血清TK1表达水平与卵巢上皮癌患者的肿瘤分化程度、FIGO临床分期、淋巴结转移有关(P<0.05);癌组织PKB阳性表达组5年内存活率(11/41,26.83%)低于癌组织PKB阴性表达组(11/19,57.89%),血清TK1高表达组5年内存活率(13/45,28.89%)低于血清TK1低表达组(9/15,60.00%),差异具有统计学意义(P<0.05);血清TK1高表达、FIGO临床分期是预测卵巢上皮癌患者术后生存期的独立危险因素(P<0.05)。结论癌组织PKB、术后血清TK1与卵巢上皮癌的临床病理参数密切相关,可对卵巢上皮癌的早期诊断及治疗提供依据。

Abstract:Objective To detect the levels of protein kinase B(PKB)and postoperative serum thymidine kinase 1(TK1)in patients with epithelial ovarian cancer,and to explore the correlations between the expression levels and prognosis.Methods 60 patients with epithelial ovarian cancer(epithelial ovarian cancer group)and 60 patients with benign ovarian tumor(benign tumor group)who were hospitalized in Zhangjiakou First Hospital from January 2013 to January 2015 and confirmed by pathology after surgery were selected as the research subjects.The level of PKB in cancer tissues was detected by immunohistochemistry,and serum TK1 expression was detected by chemiluminescence digital imaging analyzer.The relationship between PKB,serum TK1 expression and clinicopathological parameters of epithelial ovarian cancer was analyzed.The relationship between PKB and postoperative serum TK1 expression and survival rate of epithelial ovarian cancer patients was analyzed.Cox univariate and multivariate analyses were used to identify independent prognostic risk factors for epithelial ovarian cancer.Results The positive expression rate of PKB protein and postoperative serum TK1 level in epithelial ovarian cancer group were significantly higher than those in benign tumor group,with statistical significance(P<0.05).PKB and postoperative serum TK1 expression were correlated with tumor differentiation,FIGO clinical stage and lymph node metastasis(P<0.05).The 5-year survival rate of the group with positive expression of PKB in cancer tissues(11/41,26.83%)was lower than that of the group with negative expression of PKB in cancer tissues(11/19,57.89%),and the 5-year survival rate of the group with high serum TK1 expression(13/45,28.89%)was lower than that of the group with low expression of TK1(9/15,60.00%),with statistical significance(P<0.05).High serum TK1 expression and FIGO clinical stage were independent risk factors for predicting postoperative survival of patients with epithelial ovarian cancer(P<0.05).Conclusions PKB and postoperative serum TK1 are closely related to the clinicopathological parameters of epithelial ovarian cancer,which can provide a basis for the early diagnosis and treatment of epithelial ovarian cancer.
