基于虚拟现实技术的盆底肌肉锻炼对产后盆底功能障碍患者的影响

摘    要:目的探讨采用基于虚拟现实技术的盆底肌肉锻炼治疗产后盆底功能障碍患者的临床疗效。方法选取2018年1月至2019年1月北京市通州区妇幼保健院收治的110例产后半年盆底功能障碍患者作为研究对象。按随机数字法分为观察组(n=55)和对照组(n=55)。观察组采用基于虚拟现实技术的盆底肌肉锻炼,对照组采用传统的盆底肌肉锻炼,两组均治疗6个月。对比分析两组治疗前后盆底器官功能相关指标改善情况。结果两组的盆底Ⅰ类肌纤维疲劳度和盆底Ⅱ类肌纤维疲劳度比较,差异无统计学意义(P>0.05);观察组的手测阴道横断面肌力为(4.06±0.45)级、盆底Ⅰ类肌纤维肌电压为(7.86±1.62)μV、盆底Ⅱ类肌纤维肌电压为(8.03±1.62)μV,均明显高于对照组的(3.44±0.64)级、(5.16±1.22)μV、(5.58±1.32)μV,观察组压力性尿失禁、尿频、尿急、腰骶酸痛、阴道松弛和阴道干涩的发生率均显著低于对照组,差异具有统计学意义(P<0.05);治疗前,两组的PFIQ-7及PFDI-20评分比较,差异无统计学意义(P>0.05);治疗后,两组的PFIQ-7及PFDI-20评分均有降低,且观察组的评分明显低于对照组,差异具有统计学意义(P<0.05)。结论基于虚拟现实技术的盆底肌肉锻炼可以显著改善产后盆底功能障碍患者的盆底肌肉张力,有助于提高患者的产后生活质量。

Abstract:Objective To explore the clinical effect of pelvic floor muscle exercise based on virtual reality technology in the treatment of postpartum pelvic floor dysfunction.Methods 110 patients with pelvic floor dysfunction half a year after delivery admitted to Tongzhou Maternal and Child Health Hospital of Beijing from January 2018 to January 2019 were selected,and divided into observation group(n=55)and control group(n=55)according to random number method.The observation group was treated with pelvic floor muscle exercise based on virtual reality technology,while the control group was treated with traditional pelvic floor muscle exercise.Both groups were treated for 6 months.The related indicators of pelvic floor organ function indexes in the two groups before and after treatment was compared and analyzed.Results There was no statistically significant difference in pelvic floorⅠmuscle fiber fatigue andⅡmuscle fiber fatigue of the two groups(P>0.05).Hand-measured the cross-sectional muscle strength(4.06±0.45),pelvic floorⅠmuscle fibers voltage(7.86±1.62)μV,andⅡmuscle fibers voltage(8.03±1.62)μV of observation group were significantly higher than those of control group(3.44±0.64),(5.16±1.22)μV、(5.58±1.32)μV.Stress urinary incontinence,urinary frequency,urgency,lumbosacral pain,vaginal relaxation,and the incidence of vaginal dryness in observation group were significantly lower than those in the control group(P<0.05).Before treatment,there was no significant difference in PFIQ-7 and PFDI-20 scores between the two groups(P>0.05).After treatment,the scores of PFIQ-7 and PFDI-20 in both groups were decreased,and the scores of the observation group were significantly lower than that of the control group(P<0.05).Conclusions The pelvic floor muscle exercise based on virtual reality technology can significantly improve the pelvic floor muscle tone of patients with postpartum pelvic floor dysfunction,which is helpful to improve the life quality of postpartum patients.
