硝苯地平对大鼠急性胰腺炎时钙向组织内转移和低钙血症的预防作用

摘    要:目的:探讨大鼠急性胰腺炎时低钙血症的形成机理和硝苯地平对低钙血症的预防作用。方法:将130只SD大鼠随机分成4组:正常对照组(A组);手术对照组(B组);急性坏死性胰腺炎组(C组)和硝苯地平预防组(D组)。术后3,6,12h测定血清钙浓度和组织钙含量。结果;D组术后3,6,12h血清钙浓度分别为1.34,1.35,1.32mmol/L与A组接近,各组织钙含量明显降低,与C组比较差异有极显著性(P<0.01)。结论:钙向组织内转移可能是急性胰腺炎低钙血症的原因或部分原因。钙通道阻滞剂硝苯地平可阻止过多的钙进入细胞内,从而纠正低钙血症。

Abstract:Objective To explore the pathogenesis of hyp ocalcemia in acute pancreatitis and the preventive effect of a calcium channel b lockade nifedipine on hypocalcemia. Methods 130 normal healthy Sprague-Daulay rats were ran do mly divided into four groups: the normal control group (group A), the operated c ontrol (B), the acute necrotizing pancreatitis (C) and the prevention with nifed ipine (D). Experimental acute pancreatitis was induced by injecting 5 % sodium t aurocholate into pancreatic duct in rats according to Aho's method. Group D rece ived nifedipine intravenously before the induction of acute pancreatitis. Blood was collected for serum calcium examination, the cardiac, hepatic, pancreatic an d muscular tissue were drawn for tissue calcium content determination, the pancr eatic tissue were drawn for light microscopic examination, at 3, 6, 12 h after induction of acute pancrea titis. Animal mortality and average of survival time were recorded.Results At each time point after operation in the group C , the serum calcium concentration was significantly lower and the tissue calcium content was significantly higher than in the group A (P<0.01). However, at eachtime point after operation in the group D, the serum calcium concentration close d to that of the group A and the tissue calcium content was signigicantly lower than in the group C (P<0.01).Conclusion Hypocalcemia of acute pancreatitis may be the result, at least in part, of translocation of calcium from extracellular to intr acellular compartments. A calcium channel blockade nifedipine may block transloc ation of calcium from extracellular to intracellular compartments, thus correct hypocalcemia.
