矽肺患者肺阻抗图及其一阶微分图的临床观察

肺阻折图及其一阶微分图已广泛应用于临床，协助肺心病诊断和评价肺循环功能。本文检测２１０例矽肺患者肺阻抗图，发现阻抗容积图彼型多为单峰波，并出现多峰被，与健康组比较，有极显著性差异。矽肺组波幅值均随病变发展而逐渐降低，舒张指数（ｈＤ／ｈｓ）、ＲＶＥＴ亦随之下降，而ＰＥＰ及ＰＥＦ／ＲＶＥＴ则逐渐延长。因此，对评价矽肺病人肺血循环功能有一定参考价值。

Clinical Observation on pulmonary Rheogram and its First Derivative in Silicosis

The pulmonary rheogram and its firsthas been extensively used in clinic. It helps to diag-nose cor pulmonale and evaluate the function of pul-monary circulation. Through inspecting 210 cases ofsilicosis by rheography,we found the waves weremainly single-wave-crests and also multi-wave-crests,but in health group most of them were tri-wave-crests and double-wave-crests,Cornparing thetwo category of waves,P<0.001,there is a verysignificant difference. In the silicosis every categoryof the wave amplitute value(hs、hD、dz/dt)of the sili-cosis group gradually reduced with the clinical devcl-opment of silicosis,and the PEP prolonged,but theKVET shortend.The PEP and PEP/RVET gradu-ally prolonged,comparing with the healthy,there isa significant difference. We think the pulmonaryrheogram and its first derivative has a reference valuein evaluating pulmonary hemody namics and can beused as a objective index for physical examination ofsilicosis。
