膝关节滑膜炎滑膜清理术前后MMP-7与MMP-9测定和关节功能的影响分析

目的 探讨膝关节滑膜炎滑膜清理术前后基质金属蛋白酶(MMP)-7与基质金属蛋白酶(MMP)-9测定和关节功能的影响。方法 膝关节滑膜炎患者72例根据随机抽签方法分为治疗组与对照组各36例,对照组采用传统滑膜刨削手术治疗,治疗组采用关节镜下滑膜清理术。结果 治疗后治疗组的有效率是97.2%,对照组的有效率是80.6%,治疗组比对照组的疗效更为显著,差异具有统计学意义(P＜0.05)。两组术后的Lysholm功能评分明显增加,而疼痛评分明显减少,与术前对比差异明显,具有统计学意义(P＜0.05); 同时术后治疗组的Lysholm功能评分与疼痛评分与对照组对比差异也有统计学意义(P＜0.05)。术后两组滑膜组织的MMP-7与MMP-9阳性表达率都明显降低(P＜0.05),与此同时,术后治疗比对照组在MMP-7与MMP-9阳性表达率方面要明显低,差异具有统计学意义(P＜0.05)。结论 关节镜下滑膜清理术治疗膝关节滑膜炎能缓解疼痛,改善膝关节功能,降低MMP-7与MMP-9的表达,从而提高治疗综合疗效。

Effects Analysis of Joint Function and MMP-7 and MMP-9 Measurement after Synovial Arthroscopic Debridement for Knee Synovitis

Objective To investigate effects analysis of joint function and MMP-7 and MMP-9 measurement after synovial arthroscopic debridement for knee synovitis.Methods 72 patients with kneesynovitis method based on a random draw were equally divided into the treatmentgroup and the control group,the control group were given the traditional synovial planing surgery.The treatment group were given the arthroscopic debridementsynovium.Results After treatment,the response rates inthe treatment group and the control group were 97.2% and 80.6% respectively.The response rate of the treatment group were significantly better than the control group.The difference was statistically significant(P＜0.05).The postoperative Lysholm scores were increased significantly,while the pain scores weresignificantly reduced,and the postoperative Lysholm score and pain scores difference compared between the two groups were also statistically significant.Thedifference was statistically significant(P＜0.05).The postoperative synovial tissue MMP-7 and MMP-9 expression rates were in the two groups significantly lower.The difference was statistically significant(P＜0.05),while the postoperative MMP-7 and MMP-9 expression rates of the treatment group were ignificantly lower than the control group(P＜0.05).Conclusion Synovial arthroscopic debridement for knee synovitis can relieve pain andimprove knee function,reduce the expression of MMP-7 and MMP-9,thereby enhance the overall efficacy of the treatment.
