多层螺旋CT及图像后处理对复杂性骨折的诊断价值

摘    要:目的:探讨螺旋CT的不同成像方法对复杂性骨折的诊断价值。方法:应用GELightspeed16排CT机,对59例外伤患者(包括鼻骨骨折4例,复杂性颅骨骨折15例,四肢粉碎性骨折23例,椎体骨折8例,多发肋骨骨折4例,复杂性骨盆骨折5例)采用横断面扫描和三维重建,并对检查结果进行分析。59例患者中,均行X线平片检查。结果:CT图像可清晰显示骨折线,并对复杂性骨折的骨折及脱位情况了解得更清楚。通过轴位、冠状位、矢状位以及3D骨重建分析,能清晰、准确地显示骨折线,并对骨折的分离、移位及周围软组织的损伤情况均可准确地了解。结论:多层螺旋CT能立体、直观、全面地显示复杂性骨折,对临床术前诊断提供全面的信息,对术后复位效果的观察具有重要价值。

Abstract:Objective To explore the value of spiral CT in the diagnosis of the complicated fracture with the different image-forming methods. Method Axial plane scanning and three-dimensional reconstruction were performed in 59 cases of trauma patients(4 cases were fracture of nasal bone, 15 cases complicated fracture of skull, 23 cases comminuted fracture of limbs, 8 cases fracture of vertebral body, 4 cases multiple fracture of rib and 5 cases complicated fracture of pelvis), and then results were analyzed. Results CT images can clearly exhibit the fracture line, and it can be distinctly understood to the condition of complicated fracture. By axial surface, coronal section, sagittal place and 3D bone reconstruction analysis, CT picture can accurately display the fracture line, the separation and displacement of fracture and the damage state of peripheral soft tissue also can be acquired. Conclusion Multiplayer spiral CT can visually demonstrate the complicated fracture steric generally, and it is of useful in providing more information for the clinical preoperative diagnosis and in observation effect of post-operative reposition.
