26例儿童囊性颅咽管瘤的立体定向内放疗

目的 研究ＣＴ立体定向囊腔内放疗治疗儿童囊性颅咽管瘤的有效性和安全性。方法 对２６例囊性颅咽管瘤进行ＣＴ立体定向囊腔、胶体磷酸铬内治疗。结果 全部病例经手术排除囊液后临床症状迅速改善，经囊腔内放疗，１～５年随访。ＣＴ扫描显示２２例（８４．６％）瘤腔持续消失，２例（７．７％）瘤腔显著缩小，症状持续改善。２例（７．７％）分别于１、３年后因瘤实质部分增大，症状恶化而相继死亡。无手术死亡和严重并发症。结论 ＣＴ立体定向囊腔内放疗治疗儿童囊性颅咽管瘤安全、有效，可作为治疗儿童囊性颅咽管瘤首选的方法。

CT-guided stereotaxic intracavitary irradiation for cystic craniopharyngioma in 26 children

Objective To study the efficiency and safety of CT guided stereotaxic intracavitary irradiation for cystic craniopharyngioma in children. Methods Twenty six children with cystic craniopharyngioma were treated with CT guided stereotaxic intracravitary irradiation with a colloid containing chromium phosphate with 32 P. Results The clinical symptoms were improved after the evacuation of cystic fluid in all patients. No mortality or severe complications occur. After intracavitary radiotherapy and 1 to 5 years follow up, CT showed that tumor cyst gradually regressed and even disappeared in 22 cases (84.6%) , obviously decreased in 2 cases (7.7%). However, there were 2 patients died in 1 and 3 years after operation due to the enlargement of the parenchymal part of the tumors. Conclusion CT guided stereotaxic intracavitary irradiation for cystic craniopharyngioma in children is safe and effective. It should be regarded as the most appropriate surgical management.
