低浓度丝裂霉素联合羊膜移植治疗原发性青光眼的近期疗效分析

目的探讨羊膜移植联合低浓度丝裂霉素、小梁切除治疗原发性青光眼的效果。方法选择本院2011年6月～2012年12月收治的42例（60眼）原发性青光眼患者，将其随机分为研究组22例（30眼）和对照组20例（30眼），研究组患者给予小梁切除、羊膜移植联合低浓度丝裂霉素治疗，对照组患者给予小梁切除治疗，观察两组患者治疗后眼压、视力的改变情况，并统计两组患者的并发症情况。结果研究组患者术后视力改善优良率明显高于对照组，组间差异有统计学意义（P〈0．05）；两组术后1、6个月眼压均较治疗前明显降低，差异有统计学意义（P〈0．05）；研究组术后1、6个月时的眼压明显低于对照组（P〈0．05）；研究组复发率及并发症发生率明显低于对照组，差异有统计学意义（P〈0．05）。结论低浓度丝裂霉素、羊膜移植、小梁切除治疗原发性青光眼可有效改善患者视力，眼压控制满意，复发率及并发生症发生率较低，值得推广。

Short-term efficacy of low concentrations of mitomycin,amniotic membrane transplantation in treatment of primary glaucoma

Objective To investigate the efficacy of low concentrations of mitomycin,amniotic membrane transplantation in treatment of primary glaucoma. Methods 42 patients （60 eyes） with primary glaucoma in our hospital from June 2011 to December 2012 were selected and randomly divided into research group 22 patients （30 eyes） and control group 20 patients （30 eyes）,the patients in research group were given trabeculectomy,amniotic membrane transplanta- tion,low concentrations of mitomycin treatment,and the patients in control group were given trabeculectomy treatment. The intra ocular pressure （IOP）,visual changes and complications of two groups were observed after treatment. Results The excellent rate of visual acuity improvement of research group was significantly higher than thai of the control group,the difference between the groups was statistically significant （P 〈0.05）;1,6 months after treatment,the postoperative IOP of two groups was all significantly lower than that before treatment （P〈0.05）;and IOP of research group was signifi- cantly lower than that of the control group （P〈0.05）;the recurrence rate and complication rate of research group was sig- nificantly lower than that of the control group,the difference was statistically significant （P〈0.05）. Conclusion In treat- ment of primary glaucoma, low concentrations of mitomycin, amniotic membrane transplantation and trabeculectomy can improve visual acuity,and IOP control is satisfaction,the recurrence rate and complication rate is low,so it is worthy of promotion.
