穴位埋线对慢传输型便秘大鼠Cajal间质细胞的影响

摘    要:目的观察穴位埋线对STC模型大鼠肠道传输功能及结肠组织Cajal间质细胞的影响,探讨其可能作用机制及不同穴位调节STC的穴位特异性。方法将34只SD大鼠随机分为5组,即正常组(6只)、模型组(7只)、天枢组(7只)、上巨虚组(7只)及大肠俞组(7只)。其中,天枢组、上巨虚组及大肠俞组大鼠在造模成功后,分别对天枢、上巨虚及大肠俞予穴位埋线处理。继采用活性碳灌胃法测定大鼠6 h内首粒黑便排出时间、大便重量;活性碳推进试验测定碳推进百分率(%);免疫组化方法观察大鼠结肠Cajal间质细胞的表达变化。结果大鼠6 h内首粒黑便排出时间,模型组、大肠俞组(P0.01)及上巨虚组(P0.05)均高于正常组,天枢组与正常组比较差异无统计学意义(P0.05);天枢组、上巨虚组、大肠俞组均低于模型组(P0.01);上巨虚组、大肠俞组均高于天枢组(P0.01)。6 h内大便重量,模型组低于正常组(P0.01),天枢组、上巨虚组、大肠俞组均高于模型组(P0.01),上巨虚组、大肠俞组与天枢组比较差异无统计学意义(P0.05)。碳推进百分率(%),模型组、大肠俞组(P0.01)及上巨虚组(P0.05)皆低于正常组;天枢组、上巨虚组、大肠俞组均高于模型组(P0.01);大肠俞组低于天枢组(P0.05)。Cajal间质细胞阳性细胞表达数量,模型组低于正常组(P0.01);天枢组、大肠俞组(P0.01)和上巨虚组(P0.05)都高于模型组;上巨虚组、大肠俞组均低于天枢组(P0.01);大肠俞组高于上巨虚组(P0.05)。结论 Cajal间质细胞的分布减少及功能异常可能是STC的发病机制之一。而穴位埋线能通过改善结肠组织中Cajal间质细胞的表达,以达到治疗STC目的;且不同穴位调节STC存在穴位特异性,特别是天枢更具有明确治疗STC的临床选取应用意义。

Acupoint Specificity Study on the Effect of Acupoint Catgut Embedding on Cajal Interstitial Cells in STC Rats

Abstract:Objective To investigate the effect of acupoint catgut embedding on intestinal transit function and colonic Cajal interstitial cells in a rat model of slow transit constipation (STC) and explore the possible mechanisms of its action and acupoint specificity in the treatment of STC with different acupoints.Method Thirty-four SD rats were randomly allocated to five groups,namely,normal (six rats),model (seven rats),Tianshu (seven rats),Shangjuxu (seven rats) and Dachangshu (seven rats) groups.Of them after successful modeling,Tianshu,Shangjuxu and Dachangshu groups of rats were treated by catgut embedding at points Tianshu (ST25),Shangjuxu (ST37) and Dachangshu (BL25) respectively.The time to discharge the first fecal pellet in six hours was determined using a gavage of activated carbon and stool weight was measured in the rats.Carbon propulsion percentage (%) was calculated using an activated carbon propulsion test.Colonic expression of Cajal interstitial cells was examined by immunohistochemistry in the rats.Result The time to discharge the first fecal pellet in six hours was longer in the model,Dachangshu (P<0.01) and Shangjuxu (P<0.05) groups than in the normal group; there was no statistically significant difference between the Tianshu and normal groups (P>0.05); it was shorter in the Tianshu,Shangjuxu and Dachangshu groups than in the model group (P<0.01) and longer in the Shangjuxu and Dachangshu groups than in the Tianshu group (P<0.01).Stool weight in six hours was lower in the model group than in the normal group (P<0.01) and higher in the Tianshu,Shangjuxu and Dachangshu groups than in the model group (P<0.01); there was no statistically significant difference between the Shangjuxu or Dachangshu group and the Tianshu group (P>0.05).Carbon propulsion percentage (%) was lower in the model,Dachangshu (P<0.01) and Shangjuxu (P<0.05) groups than in the normal group and higher in the Tianshu,Shangjuxu an
