保留灌肠法防治放射性直肠炎的护理

目的观察康复新液保留灌肠对防治放射性直肠炎的作用。方法将80例患者随机分为观察组（康复新护理组）与对照组（常规护理组）各40例,均采用康复新液灌肠,观察组为自放疗开始后灌肠,每日1次,至常规放射治疗结束为止,出现放射性直肠炎患者灌肠处理直至症状消失。常规护理组为出现Ⅱ级肠道反应后开始灌肠,每天1次,直至症状消失。晚期出现放射性直肠炎时以同样治疗方法进行治疗,直至症状消失。结果康复新液灌肠治疗急性放射性直肠炎总治愈率82.6%,总有效率100%,治疗慢性放射性直肠炎总治愈率69.5%,总有效率91.3%。观察组急性放射性直肠炎发生率为32.5%,对照组为65%（P〈0.05）;慢性放射性直肠炎发生率观察组为17.5%,对照组为30.0%,（P〈0.05）。结论康复新液灌肠治疗放射性直肠炎效果好,且无明显不良反应,放疗期间持续保留灌肠可明显降低放射性直肠炎发生率。

Nursing of retention enema for the prevention of radiation proctitis

Objective To observe the efficacy of retention enema with new rehabilitation fluid for the prevention of radiation proctitis.Method A total of 80 patients were equally randomized into observation group（new rehabilitation nursing group） and control group（conventional nursing group）.Both groups received new rehabilitation fluid enema.The observation group received enema since radiation started,once a day to the end of conventional radiotherapy.When radiation proctitis appeared,patients received enema until symptoms disappeared.The control group started the enema when patients had level II intestinal reactions,once a day to the disappearance of symptoms.The same treatment was applied in advanced stages until all symptoms disappeared.Results The overall cure rate of enema with new rehabilitation fluid for acute radiation proctitis was 82.6% with an effectiveness rate of 100%.The overall cure rate of it for chronic radiation proctitis was 69.5% with an effectiveness rate of 91.3%.Incidence rate of acute radiation proctitis was 32.5% in the observation group while it was 65% in the control group（P〈0.05）.Incidence rate of chronic radiation proctitis was 17.5% in the observation group while it was 30% in the control group（P〈0.05）.Conclusion Enema with new rehabilitation for radiation proctitis has good outcomes with less obvious adverse events.It can significantly decrease the incidence of radiation proctitis in the course of chemotherapy.
