经纤支镜治疗急性误吸性呼吸衰竭

摘    要:目的:误吸引起的急性呼吸衰竭(ARDS)通过纤维支气管镜治疗使用气管内置管供氧技术,观察此方法的临床应用价值。方法:对12例误吸性ARDS患应用纤维支气管镜检查(FOB)进行气管内误吸物吸取,并应用经纤支镜气管内置管供氧技术监测术前、术中、术后24小时动脉血氧指标、临床症状及影像学变化。结果:应用此方法能提高思考对FOB的耐受性,使气道内异物和分泌物清除更彻底,机体低氧血症明显缓解。结论:在误吸性ARDS治疗中应用气管内置管供氧技术能对气道内异物和分泌物清除更彻底,显改善机体低氧血症,本方法操作简便,在急诊内科有一定临床推广价值。

Abstract:Objective In the treatment of acute respiratory failure induced by aspiration,we used a new method to deliver oxygen i.e placing a oxygen cathete in the windpipe via fibrobronchoscope during bronchoalveolar lavage(BAL),in this article,we discussed the curative effect to hypoxemia of this therapy.Method We used the new method during fiberoptic bronchoscopy for 12 patients with ARDS induced by aspiration,monitoring of arterial blood gas analysis was recorded at the time of before,during and 24h after fiberoptic bronchoscopy to evaluate the curative effect.Result Arterial blood gases showed that PaO 2 and pulse oximetry values (SO 2) had increased 24h after FOB than before and durin FOB(p<0.05).Conclusion In acute respiratory failure patients,the use of the new delivery of oxygen method during FOB allowed minimal alterations in gas exchange and promote the recovery of respiratory failure.
