摘    要:目的 :应用声学定量 (AQ)技术 ,分析高血压病左室舒张功能的评价指标。方法 :对照组 2 0例 ,高血压病组 51例 ,根据左室心肌重量指数和相对室壁厚度 ,将高血压病组分为左室几何构型正常组、左室向心性重构组和左室向心性肥厚组。应用 AQ技术 ,分别测量评价左室舒张功能的左房和左室 AQ指标。结果 :评价左室舒张功能的左房和左室 AQ指标显著相关 ;左室几何构型正常组的左室 AQ指标如 AFF、 RF/ AF、 F (1/ 3 ) FF等与对照组均无显著差异时 ,其评价左室舒张功能的左房 AQ指标 F (1/ 3 ) FE与 PRER/ PAER较对照组有显著减小 ,以 F(1/ 3 ) FE为著。左室 AQ指标 L V (1/ 3 ) TIME、RF/ AF、 AFF、RFF、 F(1/ 3 ) FF及左房 AQ指标 (1/ 3 ) TIME,在高血压病向心性重构组与对照组出现显著差异 ;左房 AQ指标 RE/ AE、AEF、REF,在高血压病向心性肥厚组与对照组有显著差异。结论 :左房 AQ指标对高血压病左室舒张功能异常的早期诊断具有一定意义 ,评价左室舒张功能较好的左室 AQ指标还有 L V (1/ 3 ) TIME、 RF/ AF、 AFF、 RFF、 F (1/ 3 ) FF及左房AQ指标 (1/ 3 ) TIME,其次为左房 AQ指标 RE/ AE、 AEF、 REF。

Abstract:Objective:To investigate the indices of evaluating left ventricular(LV) diastolic function in patients with hypertensio n by acoustic quantification (AQ) technique.Methods:Fifty-one hypertension patients and twenty contr ol subjects were studied According to left ventricular mass index and relative wall thickness,normal LV geometry,concentric remolding and concentric hypertroph y were grouped in patients with hypertension AQ indices of left atrium(LA) and LV which evaluate LV diastolic function were measured.Results:The correlation of indices of LA emptying with tho se of its corresponding LV filling stages were significant Compared with contr ol subjects,first third fractional emptying F(1/3)FE and ratio of peak rapid emt ying rate and peak atrial emptying rate (PRER/PAER)which were LA AQ indices to e valuate LV diastolic function were decreased significantly in patients with norm al LV geometry( P <0 05-0 01).However,there were no difference between bot h groups about atrial filling fraction(AFF),ratio of rapid filling volume and a trial filling volume(RF/AF),F(1/3)FF which were LV AQ indices to evluat LV dias tolic function.LV(1/3)TIME,RF/AF,AFF,rapid filling fraction(RFF),F(1/3)FF an d (1/3)TIME were significantly different between patients with concentric remold ing and control subjects( P <0 05-0 001).Ratio of rapid emptying volume an d atrial empying volume(RE/AE),Atrial emptying fraction(AEF),rapid emptying fr ation(REF)were just different between patients with concentric hypertrophy and c ontrol subjects( P <0 05-0 01).Conclusions:LA AQ indices are the best which can be used t o early detect LV diastolic function The better indices of evaluating LV diasto lic function were LV(1/3)TIME,RF/AF,AFF,RFF,F(1/3)FF,(1/3)TIME,and RE/AE,A EF,REF were all good indices for left atrial function.
