儿童鼻窦炎的鼻内窥镜手术治疗

目的：探讨儿童慢性鼻窦炎的特点，提高对本病的诊断及治疗水平。方法：采用鼻内窥镜鼻窦手术治疗儿童慢性鼻窦炎82例(139侧)，慢性上颌窦炎24例(42侧)，慢性筛-上颌窦炎伴中鼻甲水肿或息肉样变23例(41侧)，慢性筛一堞窦炎lO例(18侧)，慢性全鼻窦炎19例(30侧)，慢性筛窦炎4例(6侧)，慢性堞窦炎2例(2侧)。结果：总治愈率为(121／139)87．1％，好转(12／139)8．6％，无效(6／139)4．3％。3例术中出现筛骨纸样板损伤，术后出现眼睑瘀血，10例(12侧)术后出现鼻腔粘连，无大量出血及其它严重并发症。结论：鼻内窥镜鼻窦手术治疗儿童慢性鼻窦炎是行之有效的方法，且具有创伤小、操作精确、病变清除彻底等优点。

Endoscopic sinus surgery for chronic sinusitis in children

Objective: To investigate the properties of chronic sinusitis in children and to improve diagnose and treatment to this kind of disease. Methods:Eighty-two ( 139 sides) patients who suffered chronic sinusitis in children underwent endoscopic sinus surgery, including twenty-four cases (42 sides ) of chronic maxillary sinusitis, twenty-three cases (41 sides) of chronic maxillary-ethmoid sinusitis with the middle turbinate edema or polypoid tissue, ten cases (18 sides) of chronic ethmoid-sphenoid sinusitis, nineteen cases (30 sides) of chronic total sinusitis, four cases ( 6 sides) of chronic ethmoiditis, two cases ( 2 sides) of chronic sphenoidifis. Results: The cure rate was 87.1% (121/139) ,the effective rate was 8.6% (12/139), the ineffective rate was 4.3 % (6/139). Three cases of them had ethmoid paper-like fracture during the operations and eyelid blood clot after operations. Ten cases ( 12 sides ) of them had nasal adhesion after the operation. There were no severe hemorrhage and other serious complications. Conclusions: The endoscopic sinus surgery is a method of very effective, making less wound ,enabling surgical procedures more precise and eliminating diseased tissues more clean in treatments of sinusitis in children.
