16层MSCT不同重建方法在骨与关节疾病诊断中的应用价值研究

摘    要:目的:评价16层MSCT多平面重建(MPR)、三维表面遮盖法重建(SSD)和容积再现(VR)等不同重建方法在骨关节疾病诊断中的显示效果,探讨16层MSCT重建技术对骨与关节疾病的诊断与鉴别诊断价值。方法:163例骨与关节疾病患者经16层MSCT容积扫描后,在二维图像和容积数据基础上,行MPR、SSD和VR重建,结合不同病例进行不同重建方法对比研究。结果:SSD能逼真显示6例良性骨肿瘤、16例四肢长骨骨折及15例椎骨爆裂性骨折外部特征,胸、腰椎各有1例爆裂性骨折及1例良性骨肿瘤显示不佳;MPR对全部125例骨折、35例非外伤性骨病显示清晰,其中3例行MPR曲面重建,3例早期关节炎性病变显示不佳;VR对117例骨折和32例非外伤性骨病,不但能显示其外部和内部病变细节,而且能显示周围软组织受累状况,8例裂隙骨折及6例非外伤性骨病显示效果不理想。结论:VR是骨与关节疾病诊断中最全面的重建方法,MPR与SSD可优势互补,16层MSCT不同重建方法灵活综合运用,结合横断面图像,可以明显提高骨与关节疾病的诊断与鉴别诊断准确率,为临床制定治疗方案和判断预后提供更多重要信息。

Abstract:Objective:To evaluate the effect of multi-planer reconstruction(MPR),surface shaded display(SSD),the volume rendering (VR),and to investigate the value of 16-slice CT image post-process in the diagnosis of bone and joint diseases.Methods: One hundred and sixty-three patients with bone and joint diseases,which were examined by 16-slice CT,and made MPR,SSD and VR.Comparative research were did among the different process methods.Results: SSD could clearly demonstrate the fracture exterior characteristic of 6 cases benign bone tumor,16 cases with long bone fracture and 15 cases with vertebrae rupturing fracture.One thorax spinal and one lumbar spinal fracture,1 case with rupturing bone fracture and 1 case with benign bone tumor demonstration was not satisfactory;MPR could display satisfactory in all the 125 cases bone fractures,35 cases non-flesh wounds osteopathy.3 cases with early stage arthritis were not satisfactory;In 117 cases bone fractures and 32 cases non-flesh wounds osteopathy,not only demonstrated the detail,also demonstrated the soft tissue changes.Conclusion: VR is one of the best methods in the diagnosis of bone and joint disease,SSD and MPR may be supplementary reciprocally.Using the different post-process methods with original image,the diagnosis value of bone and joint diseases could be improved,and providing more valuable information for treatment plan and prognosis.
