BiPAP无创通气支持在急性左心衰治疗中的作用

目的 观察比较急性左心衰的常规药物治疗和在此基础上配合BiPAP无创双水平正压通气治疗的疗效.方法 用对比分析的方法,比较治疗组在常规治疗的同时应用BiPAP面罩双水平正压通气治疗和对照组用常规方法,即吸氧、强心、利尿、扩血管、解除气道痉挛和纠正酸碱平衡等措施治疗急性左心衰竭前、后2小时、6小时的心率、呼吸、血压、血氧饱和度、24小时西地兰用量和心功能改善等变化.结果 治疗组在常规治疗的基础上配合BiPAP无创通气2小时、6小时后的HR、R、SaO2均较对照组明显改善(P＜0.05),24小时的西地兰用量少于对照组(P＜0.05),心功能改善较对照组明显(P＜0.05).结论 常规治疗配合BiPAP无创通气对更快改善急性左心衰竭患者的生命指征、缓解临床症状、减少洋地黄用量和提高疗效起较大的作用.

The Effect of Bi-level Positive Airway Pressure Non-invasive Ventilation on the Treatment of Acute Left Ventricular Failure

Objective To observe the effect of bi-level positive airway pressure (BiPAP) non-invasive ventilation on the treatment of acute left ventricular failure(ALVF). Methods 67 patients with ALVF were divided into a BiPAP group and a control group. In addition to conventional therapy, the BiPAP group was treated with BiPAP non-invasive Ventilation. The changes of clinical symptoms, heart rate(HR), respiration rate(RR), blood pressure, SaO2 and the amounts of Lanatoside C in 24 hours in BiPAP group were compared with that in the control group before and after treatment. Results Compared with the control group, the BiPAP group showed more significant improvement in clinical symptoms, the clinical parameters of SaO2, HR, RR and fewer amount of Lanatoside C. Conclusions BiPAP non-invasive ventilation is capable to improve the vital sign and relief the clinical symptoms in patients with ALVF.
