电视胸腔镜小切口手术治疗自发性气胸52例

目的探讨电视胸腔镜辅助小切口手术（VATM）治疗自发性气胸的可行性和优越性，总结临床经验。方法回顾性分析于2003年4月～2008年1月对52例自发性气胸行电视胸腔镜辅助小切口肺大泡切除术及胸膜固定术的临床资料。结果手术时间90～260min，术后胸管留置时间3～15d，住院时间8～28d。术后肺漏气2例，复张性肺水肿2例。本组52例患者均痊愈出院。随诊6～60个月，平均33个月，无复发。结论电视胸腔镜辅助小切口手术治疗自发性气胸具有手术时间短、出血少、术后恢复快及住院时间短等优势。

Treatment of Spontaneous Pneumothorax by Video-assisted Minithoracotomy Surgery

Objective To investigate the feasibility and superiority of video assisted minithoracotomy surgery （VAMTS） for spontaneous pneumothorax. Methods The clinical data of 52 cases of spontaneous pneumothorax undergoing bullectomy and pleurodesis from Apt 2003 to Jan 2008 were analyzed retrospectively. Results The operating time was 90 to 260 minutes and the duration of chest drainage was from 3 to 15 days, the postoperative hospital stay was from 8 to 28 days. Postoperative complications were prolonged air leakage（2 cases） and pulmonary edema（2 cases）. All 52 patients were cured. No recurrence case was found in the follow-up of 6 to 60 months（ average 33 months）. Conclusion The shorter surgery time,less bleeding,faster recovery and shorter time in hospital are the main advantageous of VATM in treating patients with spontaneous pneumothorax.
