益心通脉汤治疗冠心病心绞痛血瘀证临床研究

目的观察益心通脉汤治疗冠心病心绞痛血瘀证的临床疗效，并探讨其作用机制。方法将60例冠心病心绞痛血瘀证患者随机分为2组。治疗组30例，给予益心通脉汤口服。对照组30例，给予复方丹参注射液静脉滴注。观察2组的临床疗效、心电图疗效、血液流变学各项指标。结果治疗组总有效率为90．00％，对照组总有效率为70．00％，2组差异有显著性意义（P〈0．05）。治疗组能改善心肌缺血、降低血液粘度、改善血液流变性（P〈0．05）；2组治疗后血浆内皮素1、血栓素B2均显著低于治疗前（P〈0．05，P〈0．01），6-酮前列环素有所增高，且治疗组明显优于对照组（P〈0．05）。结论益心通脉汤能改善冠心痛心绞痛血瘀证患者的临床症状，改善心肌缺血，并有改善血液流变学指标的作用。

Clinical Study of Coronary Heart Disease and Angina Pectoris with Blood Stasis Syndrome Treated by Yixin Tongmai Decoction

Objective To observe the clinical effects of Yixin Tongmai Decoction in treating angina pectoris in CHD （blood stasis syndrome） and explore its mechanism. Methods Sixty patients were randomized into two groups. The treatment group （30 cases） and the contrastive group （30 cases） were respectively given Yixin Tongmai Decoction and Danshen Injection. The clinical and ECG effect, the indexes of hemorrheology were observed. Result Effective rate of treatment group was 90.00%, significantly higher than 70.00% of contrastive group （P〈0.05）. Therapy of the treatment group has the effects of reducing blood viscosity and improving hemorrheological properties （P〈0.05）. Both two groups had the alteration of plasma endothelin （ET） level and thromboxan B2 （TXB2） significantly lowering （P〈0.05, P〈0.01） and 6-keto-prostacyclin （6-keto-PGF1α） rising, and treatment group was more remarkably alterated than contrastive group （P〈0.05）. Conclusion Yixin Tongmai Decoction has curative effect on symptoms of angina pectoris in CHD （blood stasis syndrome）, relieves heart muscle ischemia, meanwhile improves hemorrheological properties.
