乌司他丁治疗严重脓毒症的临床疗效观察

目的观察乌司他丁对严重脓毒症患者的临床疗效。方法将100例严重脓毒症患者分为治疗组(UTI组,52例)和对照组(48例),治疗组在常规治疗的基础上予以乌司他丁300 000 U静注,q 8 h×7 d。检测治疗前及治疗后第7天IL-6、TNF-α及Nt-proBNP水平,检测APACHEⅡ评分、MODS发生率。结果治疗后7d治疗组患者IL-6、TNF-α及Nt-proBNP水平较对照组明显降低(P<0.05),治疗前两组差异无统计学意义(P>0.05);治疗后7d,治疗组APACHEⅡ评分较对照组明显降低(P<0.05)、MODS发生率低于对照组(P<0.05)。结论乌司他丁辅助治疗严重脓毒症患者,可有效改善各项炎症指标,保护心、脑等器官功能,可以有效阻断MODS发生。

Clinical Efficacy of the Treatment of Severe Sepsis by Ulinastatin

Objective To study the curative effect ofulinastatin （UTI） in patients with severe sepsis. Methods A total of 100 patients with severe sepsis were randomly divided into the treatment group（n=52） and the control group （n=48）. All the patients in the two groups received routine treatment, while those in UTI group were additionally treated with 300000 U ulinastatin intravenously every 8 hours for 7 days. The IL-6, TNF-α, Nt-proBNP, APACHE- E values and the incidence of multiple organ dysfunction syndrome （MODS） were detected before treatment and on the 7th day after treatment. Results 7th day after treatment, the levels of IL-6, TNF-α, Nt-proBNP in the treatment group were reduced significantly than those in the control group （P〈0.05）. No statistically significant difference was found in the above-mentioned values before treatment between the two groups.The APACHE- E values were significantly decreased （P〈0.05）, and the incidence of MODS was significantly lower （P〈0.05）. Conclusion For patients with severe sepsis at emergency, Ulinastatin can effectively improve the inflammatory reaction, protect the important organs of patients,as well as prevent MODS.
