胰十二指肠切除术的围手术期肠内营养支持的护理探讨

目的探讨胰十二指肠切除术(pancreaticoduodenectomy,PD)围手术期应用肠内营养支持的效果及护理。方法对48例PD围手术期患者分为两组,应用肠内外营养患者22例(治疗组),单独予以肠外静脉营养患者26例(对照组),对比两组术前后体质量变化,总蛋白和白蛋白改善情况,住院时间和费用,及术前、术后并发症发生率等指标。结果治疗组手术前后总蛋白和白蛋白改善情况均优于对照组(P<0.05)。治疗组的术后并发症发生率,手术前后体质量变化及总蛋白改善情况均优于对照组(P<0.05)。结论PD围手术期应用肠内营养及加强基础护理和心理护理,预防胰漏及其他并发症的对策1],可明显改善患者的营养状况,降低术后并发症发生率,为提高生命质量创造了条件,值得临床推广。

The Effects of Enteral Nutrition on the Pancreaticoduodenectomy During the Perioperative Period

s Objectives to investigate the effects of enteral nutrition on the pancreaticoduodenectomy during the perioperative period. Methods Separate 48 cases into two groups: the treatment group of 22 patients, underwent the enteral nutrition; the control group of 26 patients, treated with total parenteral nutrition, compare the condition of these groups: weights, the improvement of protein and albumin, the length of stay in hospital, the medical charges, the complication rate. Results The treatment group is much better than the control group at the improvement of protein and albumin and the weight(P<0.05), meanwhile the complication rate of the treatment groups is lower(P<0.05). Conclusions On the pancreaticoduodenectomy during the perioperative period, treatments of enteral nutrition and strengthen of the basic nursing care and psychological care, prevention of the pancreatic leakage and other complication, it improves the nutrition condition and lower the complication rate after the operation, advances life quality, should be suggested in clinical experiences.
