丹参注射液改善新生儿缺氧缺血性脑病血流动力学的探讨

摘    要:目的:探讨丹参注射液对新生儿缺氧缺血性脑病(HIE)血流动力学的影响以及治疗HIE的作用机制。方法:74例HIE患儿随机分为治疗组和对照组,两组均常规治疗,治疗组加用丹参注射液;在生后1、2、3、7日龄用超声多普勒检测两侧大脑前动脉(A鄄CA)、大脑中动脉(MCA)和大脑后动脉(PCA)的血流动力学指标。结果:1日龄时,HIE患儿脑血流变化特点为舒张末血流速度降低,其中以左侧ACA、PCA的血流速度降低最为明显,2、3日龄时HIE治疗组患儿左侧MCA的Vs、Ved及Vm均较HIE对照组有所改善(P<0.05);7日龄时治疗组患儿两侧ACA、MCA、PCA的Vs、Ved及Vm均较对照组有明显改善。结论:HIE患儿的两侧A鄄CA、MCA、PCA血流速度普遍降低;丹参注射液能明显改善HIE患儿的脑血流动力学。

Abstract:Objective: To examine the efficacy of salvia miltiorrhiza injection in the treatment of HIE and change of cerebral blood flow dynamics. Method: 74 cases of HIE were randomly divided into treating group who were given salvia miltiorrhiza injection on the basis of comprehensive treatment and control group given comprehensive treatment. ACA, MCA and PCA blood flow dynamics indexes of two groups, obtained through ultrasonic Doppler on the 1st, 2nd, 3rd, 7th day after birth, were compared. Results: On the 1st day after birth, speed of HIE children cerebral blood flow declined, among which speed of left ACA, PCA blood flow declined most obviously. On the 2nd and 3rd day after birth, Vs, Ved and Vm in children's left side MCA in the HIE treatment group were improved (P < 0.05) compared with those of control group; On the 7th day after birth, Vs, Ved and Vm of children's MCA of both sides in HIE treatment group were obviously improved compared with those of control group. Conclusion: Blood flow of HIE children's ACA, MCA and PCA of both sides slowed down; salvia miltiorrhiza injection can obviously improve HIE children's cerebral blood flow dynamics.
