Clinical analysis of 40 cases of severe and critical influenza A H1N1

Objective To explore the the clinical characteristics and treatment experience for severe and critical influenza A H1N1. Methods 40 patients with severe or critical influenza A H1 N1 in our department of infectious among August 2009 February 2010 were involved in this study, and the clinical manifestation, laboratory examination treatment and treatment outcome were retrospectively analyzed. Results The age between the patients were from 7 to 48 years old, average in （ 17.9±2. 1 ） years old. there were 8 patients（26. 7% ） with basic diseases, 1 patients （ 3.3% ） with pregnancy. 30 patients with severe influenza A H1 N1 were treated with antiviral （Oseltamivir） ,symptomatic supportand organ protection treatment. 10 patients with critical influenza A H1 N1 were treated with antibiotics, corticosteroids anti - inflammatory, nutritional support, immune adjustment, artificial entilation and other adjuvant therapy with antiviral （Osehamivir）. All the patients were better or recovered. Conclusion The symptoms in patients with severe influenza A H1N1 progressed quickly, specifically performaneed in dyspnea and hypoxemia. Widly lesions in lungs, with basic diseases in respiratory or immunocompromised states merge bacterial infections is the risk factors for severe and critical and death in influenza A H1N1. The key of reducing mortality is that antiviral with oseltamivir, respiratory support, mix infection control and immune adjustment and other comprehensive treatment. Mechanical entilation support treatment is one of the most important methods to rescue the patients with critical influenza A H1N1.
