肩袖微创切口治疗肱骨近端骨折的疗效分析

目的 评价肩袖切口使用肱骨近端锁定钢板(LPHP)微创治疗肱骨近端骨折的临床疗效. 方法自2006年7月至2008年2月经肩袖切口入路应用LPHP治疗36例肱骨近端骨折,其中男22例,女14例.按Neer分型,其中二部分骨折5例,三部分骨折16例,四部分骨折15例.疗效评价采用Neer评分系统.结果 术后36例患者均获得随访,随访时间为18～36个月,平均26个月.根据Constant评分,优21例,良11,可4例,优良率为88.8%.结论 肩袖切口治疗肱骨近端骨折的疗效满意,这种手术方法切口暴露充分,便于肱骨近端骨折复位及肩袖修复,手术损伤小,充分体现了小切口微创治疗,是治疗肱骨近端二、三、四部分骨折的一种安全,有效的方法.

Therapeutic effects of minimally invasive approach in treating proximal humeral fracture

Objective To investigate the effect of minimally invasive treating proximal humeral fracture with plate under acromial anterior lateral deltoid splitting approach. Method A retrospective analysis on 36 patients treated with locking plate under acromial anterior lateral deltoid splitting approach from 2006,7to 2008,2 were carried out. According to Neer classification, there were 5 cases of tow - part fractures, 16 cases of three - part fractures, and 15 cases of four part fractures. Result The postoperative radiographs verified good position of all screws, with satisfactory fracture reduction. According to Constant scoring, the excellent to good result were of 88.8% of all the cases treated. Conclusion The method of minimally invasive treating proximal humeral fracture with plate under aeromial anterior lateral deltoid splitting approach is a safe and effective way to deal with proximal humeral fracture.
