摘    要:目的:探讨胎盘早剥原因及其对母儿结局影响。方法:回顾性分析平顶山市妇幼保健院2006年1月~2010年12月收治的46例胎盘早剥患者临床资料。结果:46例孕产妇胎盘Ⅰ度早剥占10.87%、Ⅱ度早剥占13.04%、Ⅲ度早剥占76.09%;胎盘早剥并发产后出血为34.78%、并发子宫卒中亦为34.78%,无孕产妇死亡;死胎率为42.55%;46例胎盘早剥患者孕周小于34周者占52.17%,孕34~37周者占34.78%,孕周大于37周者占13.05%;46例患者中妊娠期高血压疾病占41.30%、创伤占15.22%。结论:妊娠期高血压疾病和创伤是胎盘早剥的主要诱因;胎盘早剥是死胎的主要原因。

Abstract:Objective:To explore the causes of placental abruption and its effect on the mothers and their infants. Methods:The clinical data of 46 cases with placental abruption from the Pingdingshan maternal and child health hospital from January 2006 to December 2010 were analyzed retrospectively. Results:Among 46 cases with placental abruption,the proportions of degree Ⅰ placental abruption,degree Ⅱ placental abruption and degree Ⅲ placental abruption were 10.87%,13.04% and 76.09%,respectively;the proportion of cases with placental abruption combined with postpartum hemorrhage and uterine apoplexy were 34.78% and 34.78%,respectively,no maternal death occurred;the incidence of stillbirth was 42.55%;the proportions of cases with gestational weeks less than 34 weeks,34~37 weeks and more than 37 weeks were 52.17%,34.78% and 13.05%,respectively;41.30% of the cases with placental abruption were found with hypertensive disorder complicating pregnancy(HDCP);the proportion of cases combined with wound was 15.22%. Conclusion:HDCP and wound are the main precipitating factors of placental abruption,and placental abruption is a main cause of stillbirth.
