脑卒中患者抑郁状态的相关因素调查分析

目的调查分析脑卒中患者抑郁状态的发生率及相关因素。方法选择320例脑卒中患者，采用抑郁自评量表（SDS）进行抑郁诊断，汉密尔顿抑郁量表（HAMD）对抑郁程度进行评估，爱丁堡斯堪纳维亚改良版神经功能缺损评分量表（SSS）评估患者的神经功能缺损程度，并分析患者的性别、年龄、文化程度、经济状况、卒中部位、神经功能缺损程度及基础疾病等因素在PSD与脑卒中非PSD患者中的差异。结果PSD发生率为35．9％，PSD与脑卒中非PSD患者在年龄、经济状况、神经功能缺损程度、卒中部位以及合并基础疾病等方面具有统计学意义（P〈0．05或P〈0．01）。多因素Logistic回归分析显示左半球卒中、合并基础疾病是PSD的危险因素，经济条件较好是PSD的保护性因素。结论PSD在脑卒中患者中发生率较高，与患者年龄、神经功能缺损程度、脑卒中部位及合并基础疾病等因素关系密切，掌握其相关因素对于临床及时采取早期干预措施，提高患者生活质量、改善疾病预后均具有较高价值。

Investigation and analysis on the relative factors of patients with post stroke depression

Objective To investigate the incidence and related factors of patients with post stroke depression.Methods All of 320 patients with stroke were assessed by SDS,HAMD,and SSS,then we contrasted the age,gender,education background,economic position,apoplexy spot,and basic disease between the patients with and without post stroke depression.Results The incidence of post stroke depression was 35.9%,there were significantly differences in age,economic position,apoplexy spot,and basic disease between two groups(P < 0.05 or P < 0.01).Multifactorial unconditioned Logistic regression analysis showed that stroke in lift cerebral hemisphere and basic disease were risk factor of PSD,good economic position was protectant factor of PSD.Conclusion The incidence of post stroke depression is high in the patients with stroke,and is related with age,economic position,apoplexy spot,and basic disease.To study the related factors of post stroke depression has important meanings to early intervention,the quality of life and disease prognosis improving in the patients.
