奥沙利铂联合亚叶酸钙及氟尿嘧啶治疗28例晚期胃癌的临床分析

目的评价奥沙利铂联合亚叶酸钙与氟尿嘧啶治疗晚期胃癌的临床疗效和不良反应。方法对患晚期胃癌28例采用奥沙利铂＋亚叶酸钙及氟尿嘧啶方案治疗，第1天奥沙利铂130mg／m。静滴2h，第1～5天亚叶酸钙50rag／（m^2·d），氟尿嘧啶500mg／（m^2·d），静脉点滴1～1．5h，每3周重复1次为1个疗程，2个疗程结束后按WHO标准评定疗效和毒副反应。结果完全缓解1例，部分缓解11例，稳定6例，进展10例，总有效率42．8％，中位进展5．2个月，1年生存率21．7％，临床受益17例（60．7％）。骨髓抑制、周围神经感觉异常及胃肠道反应是主要的不良反应，这些不良反应均可逆，无治疗相关死亡。结论奥沙利铂联合亚叶酸钙及氟尿嘧啶治疗晚期胃癌有效率及临床受益率较高，是晚期胃癌化疗较为理想的方案。

Clinical Study on Oxaliplatin Combined with Fluorouracil and Leucovorin in the Treatment of 28 Patients with Advanced Gastric Cancer

Objective To evaluate the efficacy and toxicity of oxaliplatin plus fluorouracil/leucovorin regimen in treatment of advanced gastric cancer.Methods Twenty-eight patients with advanced gastric cancer underwent the treatment.The treatment schedule was oxaliplatin 130 mg/m2 with 500 mL of 5% Gs by drip infusion for 2 hours on first day.CF 50 mg/m2 drip infusion for one hour followed by 5-Fu 500 mg/m2 infusion which speed was 15 drops to 20 drops in a minute which continued 5 days.The chemotherapy was repeated every 3 weeks for one cycle.All patients received two cycles of chemotherapy at least.The efficacy and toxicity were evaluated according to WHO standard.Results There was one patient showing a complete response,11 part complete,6 steady disease,10 progressive disease.The total response rate was 42.8%.Median time to progression(TTP) was 5.2 months.Survival rate of one year was 21.7%.Toxicity could be assessed in all patients and the principal adverse events was neural toxicity,gastrointestinal and myolesuppression and they were reversed.There was no death during the treatment.Conclusion Oxaliplatin combined with CF/5-FU is an effective and well tolerated regimen in the treatment of advanced gastric cancer.
