摘    要:目的 研究快速成像序列动态增强对前列腺癌的检出和定性价值。方法 对经活检证实的25例,临床证实的6例,共31例前列腺癌患者作了前瞻性增强MRI研究。先行SET1WI、快速自旋回波(FSE)T2W序列扫描,然后行快速多怪面干扰梯度回波(FMPSPGR)序列团注增强扫描,共4个回合,研究病灶增强情况,并对增强后图像与常规FSET2WI进行比较。结果 FMPSPGR增强后扫描,共发现病灶29个,可疑病灶

Abstract:Objective To evaluate the value of bolus dynamic enhancement MR imaging in the detection and diagnosis of prostatic carcinoma. Methods Thirty one patients with prostatic carcinoma were prospectively studied with bolus dynamic enhancement MR imaging. Their age ranged between 52 and 81 years (average 64 years). In 31 patients, 25 cases were confirmed by biopsy and 6 cases by clinic data. The spin echo T 1 weighted image, fast spin echo T 2 weighted image, and bolus dynamic enhanced FMPSPGR images were obtained in all cases. The immediate and consecutive 3 group scans (15-20 s for each) were performed and the 4th scan was repeated in 3-5 min following injection of 18-22ml Gd DTPA (Magnevist) within 10 second. Results The dynamic enhanced MRI identified 29 abnormal lesions and 1 suspicious lesion in 31 patients, but FSE T 2WI detected only 25 lesions. In early enhanced phase, the lesions presented as hyperintense signal with different degree in 20 cases, isointense signal in 3 cases, located in peripheral zone and the contrast between lesions and surrounding parenchyma turned down in delayed phase. Slight diffuse enhancement was seen in 5 lesions of central zone. Dynamic enhanced images had same or much higher detection rate,5 lesions were identified only in dynamic enhanced images. Conclusion Fast dynamic enhanced MRI has high detection rate and improves diagnosis in prostatic carcinoma. In addition, it is also useful for differentiation from other diseases.
