内镜下套扎术联合奥曲肽治疗食管静脉曲张破裂出血的效果观察

目的 探讨内镜下套扎术（EVL）联合奥曲肽治疗食管静脉曲张破裂出血（EVB）的疗效.方法 97例肝硬化食管静脉曲张破裂出血患者,随机分为两组,实验组51例用EVL联合奥曲肽治疗;对照组46例仅用奥曲肽治疗.分别观察两组止血率、术后4、8、12周内再出血发生率.结果 实验组止血率为96.1%,对照组止血率为86.9%,两组差异有统计学意义（P 〈 0.05）;实验组术后4、8、12周内再出血发生率均低于对照组（均P ＜ 0.05）.结论 EVL联合静脉应用奥曲肽治疗EVB效果明显,可降低再出血率,值得临床推广应用.

Observation of effects of endoscopic ligation combined with Octreotide in treatment of esophageal variceal bleeding

Objective To evaluate the curative effects of endoscopic variceal ligation(EVL) combined with Octreotide in the treatment of esophageal variceal bleeding(EVB).Methods 97 patients with liver cirrhosis esophageal variceal bleeding were divided into two groups,51 patients of the experiment group were treated with EVL combined with Octreotide and 46 patients of the control group were treated with only Octreotide.The hemostasis rate and the rebleeding rates within 4 weeks,8 weeks and 12 weeks after surgery both groups were observed respectively.Results The hemostasis rate was 96.1% for the experiment group and 86.9% for the control group,with statistically significant difference(P < 0.05).The rebleeding rates within 4 weeks,8 weeks and 12 weeks after surgery of both groups were lower than those of the control group(P < 0.05).Conclusion EVL combined with the intravenous application of octreotide is obviously effective in the treatment of EVB and can reduce the rebleeding rate,thereby worthy of clinical promotion and application.
