摘    要:目的探讨恢复期缺血性脑卒中血清甲状腺激素变化与临床的相关性。方法应用放射免疫分析法(RIA)测定120例恢复期缺血性脑卒中患者甲状腺激素:三碘甲状腺原氨酸(T3)、甲状腺素(T4)、游离T3(FT3)、游离T4(FT4)和促甲状腺素(TSH)水平,并将其分为正常组76例(对照组)和异常组44例(研究组)。对每例患者评定神经功能缺损情况(美国国立卫生研究院卒中量表NIHSS)、认知功能(简易智能状态检测量表MMSE)及卒中后抑郁情况(汉密尔顿抑郁量表HAMD),并进行两组比较分析。结果两组在神经功能缺损评分、认知功能、抑郁评分方面差异均有统计学意义(P〈0.05)。结论甲状腺激素异常与恢复期缺血性脑卒中的病情严重性是相关的,应予重视。

Abstract:Objective To investigate the relationship between clinic and the changes of serum thyroid hormone level in the cerebral ischemic stroke of recovery stage.Methods 120 patients with cerebral arterial thrombosis were used the radio immunoassay method to check the thyroid hormone levels,including T3 triiodothyronine(T3),tetraiodothyronine(T4),dissociationT3(FT3),dissociationT4(FT4) and thyrotrophic(TSH).After detection,the patients were divided into 76 cases of the normal group(control group),and 44 cases of the unusual group(study group).Each patient’s nerve function(United States national institutes of health stroke scale,NIHSS),cognition function(simple smart State detection scale,MMSE) and post-stroke depression condition(hamilton depression scale,HAMD) were assessed and compared between the two groups.Results The neural function defect score,the cognitive function score and the depressive score of the two groups had significantly differences(P < 0.05).Conclusion Thyroid hormones abnormalities are related to the severity of cerebral ischemic stroke,which should be taken seriously.
