动态心电图结合颈动脉斑块超声显像与冠状动脉造影的相关性研究 |

摘 要: | 目的:探讨颈动脉斑块结合动态心电图(Holter)与冠状动脉造影(CAG)的相关性,评价超声检测颈动脉粥样硬化结合动态心电图对冠心病心脏事件预测的可能性。方法:对278例疑似冠心病的研究对象经CAG分为冠心病组和非冠心病组,均行超声观察颈动脉斑块及动态心电图结果,分析颈动脉斑块检出率、Holter、颈动脉斑块结合Holter诊断冠心病的敏感度、特异度。结果:颈动脉粥样硬化与冠状动脉粥样硬化呈明显的相关性。颈动脉斑块阳性对诊断冠心病敏感度69.2%,特异度62%,Youden指数J=0.53,Kappa值Κ=0.53。动态心电图对诊断冠心病的敏感度72.5%,特异度68.8%,Youden指数J=0.58,Kappa值Κ=0.62。Holter结合颈动脉斑块对诊断冠心病的敏感度为91%,特异度76.4%,Youden指数J=0.82,Kappa值Κ=0.80。Holter、颈动脉斑块,Holter结合颈动脉斑块检查之间敏感度、特异度比较:Holter与颈动脉斑块比较无统计学差异;Holter结合颈动脉斑块检查与Holter或颈动脉斑块检查比较,均有统计学意义。结论:颈动脉斑块检测在一定程度上诊断冠心病并且可反应冠心病的严重程度,颈部动脉斑块的存在可以预诊冠状动脉粥样硬化的存在。Holter诊断冠心病的敏感度、特异度较优于颈部动脉斑块,对诊断存在较高的一致性。Holter同时结合颈动脉斑块检查可显著提高冠心病的诊断率。

Abstract: | Objectives: To investigate the correlation of combining Holter with carotid artery plaque inspection with coronary arteriography in diagnosis of coronary heart disease.Methods:Performed coronary angiography(CAG),Holter,carotidartery plaque inspection in 278 patients of suspected coronary heart disease.CAG is the gold standard.Analyseds the sensitivity and specificity of Holter,carotid artery plaque inspection,and combining Holter with carotid artery plaque inspection in the diagnosis of coronary heart disease.Results: Carotid artery plaque inspection had correlation with coronary artherosclerosis.In turn,the sensitivity is 69.2 %,72.5 %,91 %;the specificity was 62%,68.8%,76.4%.The last was better in sensitivity and specificity in the diagnosis of coronary heart disease.Conclutions: Combining Holter with carotid artery plaque inspection can obviously improve the diagnosis of coronary heart disease,and is a good screening technique. |
