影响哮喘患者吸入性用药依从性的因素分析及对策

目的分析影响哮喘患者吸入性用药依从性的因素,寻求临床药师药学服务的切入点,为更好地防治哮喘提供依据。方法对本院2010年5月至2011年11月期间收治的105例哮喘患者进行问卷调查,按Morisky-Green(MG)标准计分评价分为依从性佳组及依从性不佳组,对影响用药依从性的因素进行统计分析。结果 105例哮喘患者中用药依从性好59例(56.19%),依从性差46例(43.81%),2组在是否正确使用吸入装置方面差异有统计学意义(P<0.01)。停药原因方面,34.78%患者认为症状缓解不需再服药,39.13%患者对治疗药物认识不够,用药指导不到位,26.09%患者遗忘用药。结论哮喘患者吸入性用药依从性不佳的主要原因为对吸入性治疗药物缺乏全面了解,临床药师应发挥其专业特长,加强对患者进行哮喘基础知识培训和吸入装置的使用及技巧的药学服务,并反复督促,以保证患者的用药依从性,从而提高疗效及患者的生存质量。

Factor analysis and countermeasures on compliance of asthma patients used inhalation device

Objective To identify the factors infl uencing the compliance of inhalation device, seek the pointcut of the clinical pharmacist service, and provide the basis for advance of asthma prevention and cure. Methods A questionnaire survey was conducted among 105 patients with asthma treated in our hospital from May 2010 to November 2011. Morisky-Green（MG） standard scoring evaluation was used for the good compliance group and the poor compliance group. Factors affecting the medication compliance was analyzed. Results All the 105 asthma patients were reviewed： 59（56.19%） had good compliance and 46（43.81%） had poor compliance. There was a signifi cant difference between the 2 groups in correct understanding of the inhalation therapy drugs. The reason of poor compliance was that 34.78% of the patients considered it an unnecessary continuance for the relieved symptoms, 39.13% lacked instruction of inhalation of therapy drugs and 26.09% forgot. Conclusion The main factor for poor compliance is the lack of comprehensive understanding of the inhalation therapy drugs. Clinical pharmacists should play their professional expertise, help patients with the training and use of inhalation devices, repeatedly supervise patients＇ compliance and fi nally improve patients＇ quality of life.
