无创正压通气不同EPAP水平治疗重度充血性心衰的疗效观察

目的：评价无创双水平气道正压通气（BiPAP）不同的呼气相正压（EPAP）水平治疗重度充血性心衰的疗效。方法：43例重度充血性心力衰竭患者随机分为观察组（BiBAP通气EP-AP〈3cmH2O）（19例）和治疗组（BiPAP通气EPAP5-10cmH2O）（24例）,观察两组患者临床疗效和治疗前后的血气分析与心功能指标的改善情况。结果：治疗组有效率高于观察组,差异有显著性;治疗组氧分压（PaO2）、氧饱和度（SaO2）、心搏量和射血分数均明显高于观察组,二氧化碳分压（PaCO2）明显低于观察组,差异有显著性。结论：无创双水平气道正压不同EPAP水平对重度充血性心力衰竭的治疗效果有差别,适度低水平EPAP能提高充血性心衰的治疗效果。

Therapeutic Effect of Noninvasive Bi-level Positive Airway Pressure Ventilation in Different Levels of EPAP for Severe Congestive Heart Failure

Objective：To research the therapeutic effect of non-invasive bi-level positive airway pressure ventilation（BiPAP）in different levels of EPAP for severe congestive heart failure.Method：43 cases with severe congestive heart failure were randomly divided into observation group（BiBAP,EPAP3cmH2O）（n=19）and treatment group（BiPAP,EPAP5-10cmH2O）（n=23）.Then observed two groups＇ therapeutic effect and the differences between the results of cardiac function and blood gas analysis before and after the treatment.Result：The therapeutic effect of the treatment group was much greater than that of observation group,and the difference was significant.Partial pressure of oxygen（PaO2）,oxygen saturation（SaO2）,stroke volume and ejection fraction in the treatment group were significantly higher than those in the observation group.Partial pressure of carbon dioxide（PaCO2）in treatment group were significantly lower than those in the observation group.Conclusion：The therapeutic effect of non-invasive bi-level positive airway pressure ventilation with different levels of EPAP for severe congestive heart failure are different,propriate low EPAP can improve the treatment＇s efficiency for severe congestive heart failure.
