营养支持对有营养风险病人临床结局的影响

摘    要:目的探讨合理的营养支持对有营养风险病人临床结局的影响。方法选择新入院病人557例,根据营养风险筛查2002进行营养风险筛查,记录病人住院期间营养支持应用情况及住院时间,对感染性、非感染并发症及营养支持并发症的发生情况进行监测。结果 557例病人中,无营养风险者310例,总并发症发生率明显低于有营养风险病人(χ2=41.427,P〈0.01)。存在营养风险病人247例中,营养支持组总的并发症发生率与无营养支持组比较,差异无显著性(P〉0.05);感染性并发症发生率低于无营养支持组(χ2=4.864,P〈0.05);非感染性并发症发生率高于无营养支持组(χ2=6.803,P〈0.05);营养支持相关并发症发生率差异无显著性(χ2=2.354,P〉0.05)。无营养风险病人中,有营养支持病人与无营养支持病人之间总并发症、非感染性并发症发生率无明显差异(χ2=0.377、3.259,P〉0.05),而感染性并发症及营养支持相关并发症发生率增加,差异有显著性(χ2=4.414、6.292,P均〈0.05)。存在营养风险病人应用营养支持并未缩短住院时间(t=2.354,P〉0.05)。结论有营养风险病人应用营养支持可以使病人受益。

INFLUENCE OF NUTRITIONAL SURPORT ON CLINICAL OUTCOME IN PATIENTS WITH NUTRITIONAL RISK

Abstract:Objective To observe the influence of a reasonable nutritional support on clinical outcome in patients with nutritional risk. Methods This study consisted of 577 new inpatients who were assessed according to standard of NRS 2002.Their nutritional support and length of hospital stay were recorded;the infected,non-infected,and nutritional-support(NS) complications were monitored. Results Of the 557 patients,the total complication rate in 310 with no nutritional risk was obviously lower than patients with nutritious risk(χ2=41.427,P0.01).In 247 with nutritional risk,the difference of total complication rate between NS and non-nutritional-support(NNS) groups was not significant(χ2=6.803,P〉0.05);the infected complication rate in NS group was lower(χ2=4.864,P〈0.05),and the non-infected complication rate was higher than the NNS(P〈0.05);the nutrition-related complications were not significantly different between the two groups(P〉0.05).In patients with no nutritional risk,the differences between NS and NNS groups were not significant in terms of total complications and non-infected complications(χ2=0.377,3.259;P〉0.05),and infected complications and NS-related complications increased(χ2=4.414,6.292;P〈0.05).The hospital stay was not shortened in the patients with nutritional risk after NS therapy(t=2.354,P〈0.05). ConclusionApplication of nutritional support would benefit patients with nutritional risk.
