老年高血压患者颈动脉结构及血流动力学变化的彩色多普勒超声检测价值

目的探讨老年高血压患者颈动脉结构及血流动力学改变的彩色多普勒超声检测价值。方法应用彩色多普勒超声检测86例老年高血压患者（高血压组）及40名健康老年人（对照组）颈动脉内径、内膜中层厚度（IMT）、斑块发生率、血流收缩期峰值流速（PSV）、阻力指数（RI）及最大剪切率（SR）,分析比较高血压组与对照组以及高血压1、2、3级亚组间上述指标的差异。结果老年高血压组颈动脉IMT、斑块发生率明显高于对照组,PSV、SR低于对照组,2组比较差异有统计学意义（均P〈0.01）;高血压1、2、3级亚组之间随着高血压分级升高,颈动脉IMT、斑块发生率也逐渐升高,PSV、SR进一步降低,组间比较差异有统计学意义（均P〈0.05）。结论老年高血压患者存在颈动脉结构及血流动力学改变,且颈动脉硬化程度与血压升高程度呈正比。应用彩色多普勒超声检测颈动脉粥样硬化的发生与发展对预测高血压患者发生心、脑血管事件具有重要意义。

Detection value of color Dopplar ultrasound for structure and hemodynamic changes of carotid artery in elder patients with hypertension
