原发性子宫颈小细胞癌6例临床病理研究

目的探讨原发性宫颈小细胞癌的临床病理特征和免疫表型。方法对6例宫颈小细胞癌进行组织病理学观察和免疫组化检测,并复习所有临床资料。结果6例子宫颈小细胞癌患者中单纯小细胞癌4例,伴腺样分化1例,伴腺鳞癌及宫内膜腺鳞癌1例。临床表现为阴道不规则出血伴宫颈肿物。组织学上肿瘤主要由大小一致的小卵圆形细胞组成。免疫组化:5例肿瘤细胞CK和Syn( ),4例CgA和CD99( ),1例5-HT( ),3例HPV( )。随访4例死亡。结论宫颈小细胞癌是一种罕见的高度恶性肿瘤,其发生可能与HPV感染有关,CK、CgA和Syn是宫颈小细胞癌较为特异的免疫标记物。

Clinicopathologic features of primary small cell carcinoma in cervix

Objective To study clinical pathologic features and immunophenotype of primary small cell carcinoma in the cervix. Methods Six cases of primary small cell carcinoma in the cervix were studied by morphologic and immunohistochemical techniques and relative clinical data were reviewed. Results The patients were between 32 and 52 years of age(average 44 years) at diagnosis who presented with symptoms of viginal bleedings with cervical masses.Microscopically,all tumors were composed of small-sized cells in uniformity.Four of six cases were pure small cell carcinoma,one was with glandular-like differentiation,one case was mixed with adenosqumaous carcinoma of the cervix and adenosqumaous carcinoma of endometrium.Immunohistochemically,the tumors were immunoreactive to CK and Syn(five cases),CgA and CD99(four cases),5-HT(one case) and HPV(three cases).Follow-up revealed death in all four patients. Conclusions Primary small cell carcinoma of the cervix is a rare highly aggressive neoplasm and its carcinogenesis may be correlated with HPV16/18 infection.CK,CgA and Syn may be useful markers in the diagnosis of primary small cell carcinoma in the cervix.
