糖尿病足患者血管内皮相关因子与足背动脉血流动力学相关性研究

摘    要:[目的]研究糖尿病足患者血清同型半胱氨酸(Hcy)及一氧化氮(NO)与足背血流动力学间的关系。[方法]分别测定糖尿病足患者(A组)、糖尿病无合并下肢血管病变患者(B组)、健康体检者(健康对照组)Hcy,NO水平,及足背动脉血管内径(D)、峰值流速(Vmax)和血流量(Vol)及频谱宽度(W),对比观察血管内皮相关因子与足背动脉血流动力学指标间关系。[结果]A组血清Hcy水平较B组及健康对照组增高,NO水平则降低(P<0.01);与B组及健康组比较,糖尿病足患者可见足背动脉D变小,Vmax加快,Vol减少,W增宽(P<0.05或P<0.01);A组Hcy与Vmax呈正相关(P<0.05),与Vol呈负相关(P<0.05);A组NO与Vmax呈负相关(P<0.01),与Vol呈正相关(P<0.05)。[结论]糖尿病足患者存在血管内皮功能下降及足背动脉硬化,且两者存在一定关系。

Abstract:[Objective] To investigate the relation between homocysteine(Hcy) and nitric oxide(NO) with dorsal arterial hemodynamics in patients with diabetic foot.[Methods](1) The levels of Hcy and NO were detected in diabetic patients with(group A) or without(group B) diabetic foot and health subjects(control group);(2) Dorsal arterial diameter(D),maximum blood flow velocity(Vmax),blood flow volume(Vol) and spectral width(W) were also detected.Results from(1) and(2) were compared to determine whether there were any links between the vascular endothelial factors and the hemodynamic indicators of dorsal artery of diabetic foot.[Results] When compared with group B and the control group,group A showed an increased level of Hcy and a decreased level of NO(P<0.01),and shorter D,faster Vmax,reduced Vol and expanded W(P<0.05,or P<0.01).In group A itself,Hcy was positively related to Vmax(P<0.05) and negatively related to Vol(P<0.05);NO had a negative relation with Vmax(P<0.01) and a positive relation with Vol(P<0.05).[Conclusions] Decreased vascular endothelial function is associated with dorsal arterial sclerosis in patients with diabetic foot.
