肺表面活性物质预防早产儿肺透明膜病的临床研究

目的探讨外源性肺表面活性物质(PS)预防早产儿肺透明膜病(HMD)的有效性及临床价值。方法对19例高危早产儿出生后予气管内滴入单剂量PS(Curosurf100mg/kg),并与同期未用PS的21例高危早产儿进行对比研究,观察其临床表现、血气指标、住院时间、HMD发生率等。结果预防组早产儿其临床症状、血气指标明显改善,氧疗时间(9.3±7.1)d,机械通气时间(2.4±3.1)d,住院天数(39±16.3)d,而对照组氧疗时间(12.8±6.8)d,机械通气时间(4.5±3.3)d,住院天数(51.5±26.3)d。预防组HMD发生率15.78%,对照组HMD发生率33.33%,两组比较差异均有统计学意义(P<0.05)。预防组3d死亡率为0,较对照组(9.52%)低,两组比较差异有统计学意义(P<0.05)。结论应用外源性PS对早产儿进行预防性给药可改善早产儿临床症状,减少HMD发生,缩短住院时间,提高早产儿存活率。

Clinical study on preventing premature infant hyaline membrane disease with pulmonary surfactant

Objective To study the efficacy and clinical value of pulmonary surfactant on premature infant's hyaline menbrace disease.Methods Nineteen high risk premature infant's were received tratment of PS hy injected into trachea in 5 minutes of life at dose of 100mg/kg and were compared with twenty-one cases of same condition,contrasted group received treated without pulmonary surfactant injected.Results The clinical symptoms and results of blood gas analysis in prevented group were improved.The time of inhaled oxygen and mechanical ventilation,and the time of inpatient in prevented group were significantly shorter than the contrasted group(P<0.05),the rate of HMD and the three-day deathed rate in prevented group were lower than the contrasted group.Conclusion The therapy prevented for premature by pulmonary surfactant may improve clinical symptom of premature,reduce the rate of hyaline membrane disease,shorter the time of inpatient and increase livability of premature infant.
