摘    要:目的:观察东莨菪碱干预治疗新生儿缺氧缺血性脑病(HIE)的临床预后。方法:跟踪观察使用东莨菪碱的114例HIE患儿(干预组)和未使用东莨菪碱的85例惠儿(对照组)的行为神经测定(NBNA)评分、智商(IQ)和发育商数(DQ)。结果:NBNA测定4-6d龄后干预组明显高于非干预组。这种差距一直保持至3个月;IQ测定中、重度HIE惠儿分别在6个月、12个月仍有显著差异;DQ测定两组大运动和精细运动在4周有显著性差异。两组适应能力及语言的差异无显著性;两组社交能力在16周开始差异有显著性。结论:东莨菪碱干预疗法可以使HIE惠儿脑组织及脑功能在前6个月得到充分改善,减少了HIE后遗症。

Abstract:Objective: To study effects of early intervention by scopolamine on neonates with hypoxia-ischemic encephalopathy. Methods: To determine the scores of NBNA, IQ, DQ and the result by following up 114 neonates with HIE who have been treated by scopolamine and 85 neonates with HIE who have not. Results: The score of NBNA of the intervention group of 4 ~ 6 days is obviously higher than that of none intervention group. This gap lasted for 3 months, especially in severe and medium cases. As for IQ assessment, there is a remarkable difference within 6 months in medium HIE. But the remarkable difference continue to 12 months in severe HIE cases. DQ assessment showed gross and accurate movements in 4 weeks are remarkable different between the two groups. The adaptation and speech abilities between the two groups are not distinct. The ability of social contact between the two groups is obviously not the same after 16 weeks. Conclusion: During treating infants with HIE, scopolamine can make the cerebral functions improved at the key age, alleviate cerebral ischemia injury obviously and reduce the rate of sequelae.
