胫后血管肌间隙支为蒂胫骨内侧骨膜瓣移位术的临床应用

目的：为治疗胫骨中、下段骨折骨不连、骨缺损提供新的术式。方法：在４０侧成人下肢标本上，观测了胫骨内侧面骨膜血管来源、走行、分支分布及其吻合，并设计以胫后血管肌间隙支为蒂的胫骨内侧骨膜瓣转位术。结果：通过解剖学研究，应用该骨膜瓣移位治疗胫骨中、下段骨折骨不连、骨缺损１２例，胫骨骨皮缺损１例，均获得良好效果。结论：术式解剖容易，血供可靠，适用于修复胫骨中、下段骨不连，骨缺损。

The transfer of medial tibial periosteal flap with the intermuscular septum branches of posterior tibial vessels

Objective:To provide a new operative method for repairing fracture,non-union and bone defect in middle or distal tibia.Methods:The origin,course,branch,distribution and anastomosis of the artery to the medial tibial periostum were oberved on 40 lower limbs of adult cadavers.The transposition of tibial periostel flap was designed with the intermuscular septum branches of posterior tibial vessels.Results:Based on anatomical study,13 cases of non-union and bone defect in the middle or distal tibial region were treaded by transposition of this periosteal flap.Conclusions:The periosteal flap is rich of blood supply and easy for surgical operation in repairing fractures,non-unions or bone defects in the middle or distal tibial region.
